Purpose: The purpose of this study was to analyze the relationship between lower urinary tract symptoms and urodynamic parameters to investigate the characteristics of mixed urinary incontinence (MUI) and stress urinary incontinence (SUI). Methods: The subjects were 318 women with MUI and 128 women with SUI. Data were collected retrospectively from electronic medical records including Bristol Female Lower Urinary Tract Symptoms-Scored Form (BFLUTS-SF), Incontinence Quality of Life Instrument (I-QOL), voiding diaries, and urodynamic parameters. Results: Compared with the SUI group, the MUI group was older and showed lower I-QOL and more severe urinary tract symptoms. The MUI group had more urinary frequency, more nocturia, and a higher urgency score than the SUI group. In the correlation analysis, the greatest difference between the two groups was that urgency was associated with Qmax, maximal cystometric capacity, and detrusor overactivity only in the MUI group (r = -.175, p= .004; r = -.281, p< .001; r= .232, p< .001, respectively). Conclusion: As a result of this study, we propose that a customized management program that emphasizes the control of urgency for the MUI group, and one that effectively strengthens the weak pelvic floor for the SUI group.

제안 방법

대상 데이터

본 연구 대상자는 2013년 1월 1일부터 2014년 8월 31일까지 서울 소재 S종합병원의 비뇨기과에서 요역동학검사를 실시한 성인 여성중에서 다음과 같은선정 조건에 근거하여 편의표집하였다.
이와 같은 연구 대상자 선정조건에 적합한 492명을 편의표집하였고, 이 중에서 자가설문지에서 미응답 문항이 많거나, 배뇨일지를 작성하지 않은 경우를 제외한 총 446명을 최종 대상자로 하였고 다음의 조작적 정의에 따라 복합성 요실금군과 복압성 요실금군으로 나누었다.

데이터처리

1) 복합성 요실금군과 복압성 요실금군의 일반적특성, 삶의질, 하부요로증상에 따른 차이는 Chi-square test 또는 t-test로 분석하였다.
2) 복합성 요실금군과 복압성 요실금군의 배뇨일지, 요역동학 검사 결과의 차이는 t-test, Chi-square test, Fisher’s exact test로 분석하였다.
3) 복합성 요실금군과 복압성 요실금군의 요실금 관련 변인과 요역동학 검사결과간의 상관관계 분석은 Pearson’s correlation으로 분석하였다.

이론/모형

여성의 하부요로증상을 조사하기 위해 Brooks 등[12]이 개발한 Bristol Female Lower Urinary Tract Symptoms-Scored Form (BFLUTSSF)을 Oh 등[13]이 한국어로 번역하고 타당도를 검증한 도구를 사용하였다, 저장증상 4문항(0-15점), 배뇨증상 3문항(0-12점), 실금증상 5문항(0-20점), 성기능 2문항(0-6점), 삶의 질 5문항(0-18점)의 5개 영역으로 구성되어 있으며, 점수가 높을수록 증상이 심각함을 의미한다. Brooks 등[12]의 연구에서 Cronbach’s α값은 ‘저장증상’ 영역이 .
요실금에 대한 삶의 질을 측정하기 위해 Patrick 등[14]이 개발한 Incontinence Quality of Life Instrument (I-QOL)을 Oh 등[15]이 한국어로 번역하고 타당도를 검증한 도구를 사용하였다. 기피 또는 제한행동 8문항, 정신사회적 영향 9문항, 사회적으로곤란한상황 5문항의 3개 영역으로 구성되어 있으며, 5점 척도로 점수가 높을수록 요실금에 대한 삶의 질이 높음을 의미한다.

성능/효과

본 연구에서 복합성 요실금군이 복압성 요실금군에 비해 나이가 많고 주관적으로 느끼는 요실금 정도가 심했고, 삶의 질이 저하되어 있었으며, 요실금을 포함한 모든 하부요로증상이 심했다. 배뇨일지와 요역동학 검사결과에서 복합성 요실금군은 복압성 요실금군에 비해 배뇨횟수와 야간뇨 횟수가 많고, 요절박이 심했으며, 잔뇨가 많고 요누출압과 최대요도폐쇄압이 낮았다. 요실금 증상과 요역동학 검사결과간의 상관관계를 분석한 결과, 두 군 모두 최대요속과 최대방광용적에 대해 배뇨횟수는 음의 상관관계, 기능적 방광용적은 양의 상관관계를 나타냈다.

이상의 결과를 종합하면 두 군 모두 최대요속을 증가시키기 위해서는 배뇨횟수를 감소시키는 전략이 필요하며, 특히 대상자의 삶의 질과 하부요로증상이 더 저하되어있는 복합성 요실금군에서는,복압성 요실금군과 차별되는 의미있는 변인인 야간뇨, 3점 이상의요절박, 요저장 점수의 호전을 통해 최대요속을 증가시킬 수 있음을 알 수 있었다. 또한 최대방광용적에 대한 상관관계에서도 복합성 요실금군과 복압성 요실금군이 유일하게 다른 변인은 복합성 요실금군에서 3점 이상의 요절박이었으므로, 복합성 요실금군은 요절박을 조절하는 것이 증상 완화의 관건이라고 본다.

후속연구

복압성 요실금군과 다르게 복합성 요실금군에서만 나타난 특성으로, 대상자의 3점 이상의 요절박 변인은 요역동학 검사결과의 최대요속, 최대방광용적에 음의 상관관계를, 배뇨근 과활동성과 양의상관 관계를 나타냈다. 따라서 본연구결과를 토대로 복합성 요실금군에게는 요절박 조절을 우선으로 하고 약해진 골반저를 강화시키는 전략으로 접근하며, 복압성 요실금군에서는 골반저근운동을 효과적으로 실시하는 전략의 간호 중재 프로그램을 개발하고 그 효과를 확인하는 연구를 제언한다. 또한 본 연구결과는 일 종합병원의 환자들을 대상으로 조사한 것이므로 향후다수기관의 더 많은 환자들을 대상으로 하는 반복적인 연구가 필요하며, 요실금을 유형과 중증도에 따라 분류하여 요역동학 검사결과와의 상관관계를 알아보는 연구도 실시해 볼 것을 제언한다.
