Purpose: This study was performed to identify the impact of aggressiveness of patients and a sense of coherence on posttraumatic stress in psychiatric nurses. Methods: After collecting data from 162 psychiatric nurses, we carried out a t-test, ANOVA, Pearson's correlation coefficient, and multiple regression using IBM SPSS Statistics 19.0. Results: The mean score of posttraumatic stress was $20.75{\pm}16.59$ points. Verbal aggression, aggressiveness about property, aggression toward oneself, and aggression toward others had a positive correlation with posttraumatic stress, while a sense of coherence had a negative correlation with post-traumatic stress. It was concluded that the significant predictors of posttraumatic stress in psychiatric nurses were aggression toward oneself, a sense of coherence, and aggression toward others, all of which accounted for 38.9% of the variability. Conclusion: The results of this study indicate that the factors influencing posttraumatic stress in psychiatric nurses were aggression toward oneself, aggression toward others, and a sense of coherence. Therefore, education programs should be developed in consideration of the fact that aggressive behavior against the patient himself and against others intensify the posttraumatic stress of the psychiatric nurse, but the integration force mitigates it.

90로 산출한 결과157명이었으므로, 탈락률을 고려하여 171명을 목표대상자로 설정하였다. 이 중 응답이 불완전한 9명의 자료를 제외한 162명(94.7%)의 자료를 최종 자료 분석에 이용하였다. ;

외상 후 스트레스는 Horowitz, Wilner와 Alvarez (1979)가 개발하고 Weiss와 Marmar (1997)가 수정 ․ 보완한 Impactof Event Scale-Revised version (IES-R)을 Eun 등(2005)이번안한 한국판 사건 충격 척도 수정판(IES-R-K) 22문항으로 측정하였다. 이 도구는 과각성 증상 6문항, 회피증상 6문항, 침습증상 5문항, 수면장애 및 정서적 마비, 해리증상 5문항의 4가지 하위개념으로 구성되어 있으며, 최소 0점에서 최대 4점의5점 리커트 척도로 25점 이상은 외상 후 스트레스 장애가 있음을 의미한다.
통합력은 Antonovsky (1987)가 개발한 13문항의 통합력 측정도구를 누가 번역한 도구를 Kim (2013)이 번안하여 내용타당도 검증을 거친 척도 13문항으로 측정하였다. 이 도구는 ‘당신은 주변에서 일어나는 일에 대해 관심을 갖지 않는다고 느끼십니까’ 등의 이해력 5문항, ‘자신이 친숙하지 않은 상황에 있고, 무엇을 할지 모른다고 느끼십니까’ 등의 관리력 4문항, ‘당신이 일상에서 하는 일이 의미가 없다는 느낌을 자주 갖습니까’ 등의 의미부여 4문항의 하위개념으로 구성되어 있으며, 최소 1점에서 최대 7점의 7점 리커트 척도이고 점수가 높을수록 통합력이 높은 것을 의미한다.
환자의 공격행동은 Yudofsky등(1986)이 개발하고 Silver과 Yudofsky (1991)에 의해 타당도 및 신뢰도가 입증된 공격성에 대한 객관적인 평정척도인 OAS (Overt AggressionScale)를 Kim과 Lee (1992)가 번안한 척도 16문항으로 측정하였다. 이 도구는 언어적인 공격행동 4문항, 사물을 대상으로 하는 공격행동 4문항, 자기 자신을 대상으로 하는 공격행동 4문항, 다른 사람을 대상으로 하는 공격행동 4문항의 하위개념으로 구성되어 있다.

성능/효과

대상자가 경험한 환자의 언어적인 공격행동, 사물을 대상으로 한 공격행동, 자신을 대상으로 한 공격행동, 다른 사람을 대상으로 한 공격행동 및 통합력을 예측변수로 하여 다중회귀분석을 실시한 결과, 환자 자신을 대상으로 한 공격행동(β=.37,p<.001), 다른 사람을 대상으로 한 공격행동(β=.26, p<.001) 및 통합력(β=-.30, p<.001)이 외상 후 스트레스에 영향을 주는 것으로 나타났고, 대상자의 외상 후 스트레스에 대한 이들 변수의 설명력은 38.9%였다.
첫째, 정신과 병동 간호사의 외상 후 스트레스는 총 88점에 평균 20.75점으로 외상 후 스트레스 장애를 진단하는 기준점인 25점 이상을 보인 대상자가 34.6%였다. 이러한 결과는 본 연구와 동일한 연구대상자와 측정도구를 사용하여 얻은 외상 후 스트레스에 대한 선행연구가 없어 정확한 비교는 어려우나, 일본정신과 병동 간호사의 외상 후 스트레스를 측정한 연구(Inoue,Tsukano, Muraoka, & Okamura, 2006)에서 언어적인 공격행동에 의한 외상 후 스트레스만 측정하였을 때 대상자의 21.

넷째, 본 연구결과를 바탕으로 하여 환자 자신을 대상으로 한 공격행동과 다른 사람을 대상으로 한 공격행동을 경험한 정신과 병동 간호사의 정서적 안녕상태에 관심을 가지고, 통합력을 향상시킬 수 있는 프로그램을 제공하여, 외상 후 스트레스에 미치는 효과를 검정하는 연구를 제안한다.
특히, 정신과 병동 간호사가 경험한 환자 자신을 대상으로 한 공격행동은 정신과 병동간호사의 외상 후 스트레스를 설명하는데 있어 가장 큰 영향을 미치는 변인이었고, 다른 사람을 대상으로 한 공격행동 또한 정신과 병동 간호사의 외상 후 스트레스에 영향을 주는 변인이었다. 따라서 이러한 환자의 공격행동을 경험한 정신과 병동 간호사의 외상 후 스트레스 증상을 조절하고 병리적인 상태인 외상 후 스트레스 장애를 예방하기 위해서는 정신과 병동 간호사의 심리적, 정서적 상태에 대한 정확한 진단이 필요하며 간호 관리자는 정신과 병동 간호사의 안녕을 위한 행정적, 제도적 차원의 대처방안을 마련해야 할 것이다. 또한 높은 통합력은 정신과 병동 간호사의 외상 후 스트레스를 조절할 수 있는 긍정적 자원이므로 이를 향상 시킬 수 있는 중재 방안을 마련하는 것이 효과적일 것이다.
