Purpose: The purpose of this study was to develop a simulation module for teaching home health care and evaluate the applicability of the program to nursing students' practical training. Methods: The simulation module was developed based on the National League for Nursing Jeffries Simulation Theory. The theme of the developed scenario was teaching nasogastric tube feeding to the caregiver of patient with Parkinson disease. Participants were 61 nursing students who had learned tube feeding, and participated in the questionnaire survey after the simulation training. Results: The evaluation of simulation design showed the highest score on feedback/guided reflection, and was highly evaluated in the order of objectives/information, problem solving and fidelity. The educational practice of the simulation was highly evaluated in the order of active learning, high expectation and diversity of learning. The nursing students showed high satisfaction and self-confidence after the simulation education. Conclusion: We suggest that the developed simulation module can be applied to practical training for home health care. In the future, the change of self-efficacy, clinical judgment and performance ability of the students after the simulation education should be identified. Also, various simulation modules related to the community health nursing competencies should be continuously developed and verified.

간헐적 위관영양 실습평가의 학습목표를 성취한 4학년 간호학생 61명의 간호학생을 대상으로 미국간호연맹 제프리스 시뮬레이션 이론에 근거하여 개발한 ‘비위관을 삽입한 재가 파킨슨 환자를 돌보는 돌봄제공자에 대한 비위관 관리교육’ 시뮬레이션 모듈을 활용한 지역사회간호학 실습교육을 실시한 후, 시뮬레이션 모듈의 적절성과 시뮬레이션 교육의 효과를 평가하였다. 시뮬레이션 모듈의 적절성은 시뮬레이션의 설계 요소를 평가함으로써 확인하고자 하였고, 시뮬레이션 교육의 효과는 시뮬레이션 중 일어나는 활동에 대한 교육적인 측면과 학습에 대한 만족도와 자신감을 평가함으로써 확인하고자 하였다.

대상 데이터

개발한 시뮬레이션 모듈과 교육의 효과를 평가하기 위한 연구의 대상은 서울시 소재 일 대학교 간호학과 4학년 학생 중 지역사회간호학 실습에 참여한 61명이었다. 대상자는 모두 시뮬레이션 교육에 참여한 경험이 있었고 실습 전에 시나리오 수행에 필요한 간헐적 위관영양 간호술기 실습을 완료한 자로 하였다.

데이터처리

이후 자의로 설문조사에 참여하기를 희망하는 학생에게 서면참여 동의서를 받고 설문지를 작성하도록 하여 자료를 수집하였다. 수집된 자료는 SPSS/WIN 23을 이용하여 빈도와 백분율, 평균과 표준편차를 산출하였다.

이론/모형

시뮬레이션 모듈을 활용한 실습에서 모듈을 평가하고 실습교육의 효과를 평가하기 위해 미국간호연맹 시뮬레이션 혁신 자원센터(NLN Simulation Innovation Resource Center)에서 개발한 시뮬레이션 평가도구를 사용하였다. 이 도구는 내용 타당도가 검증된 것으로 시뮬레이션 설계를 평가하는 도구와 시뮬레이션의 교육적인 측면을 평가하는 도구 및 학습자의 반응을 평가하기 위한 도구로 구성되어있다[21,22].

성능/효과

연구결과 개발한 시뮬레이션 모듈은 학습목표를 성취하고 문제를 해결하는데 적합할 뿐 아니라 피드백/성찰 요인이 높은 수준을 보여 디브리핑에 대한 설계가 잘 되어 활용가능성이 확인되었다. 또한 시뮬레이션 교육이 교육적 측면의 효과 평가 요인인 동료와의 협력, 능동적 학습, 다양성, 기대감의 모든 항목에서 점수가 높았고, 학습에 대한 만족도와 자신감 요인 모두 높은 점수를 보여 개발한 시뮬레이션 모듈을 적용한 실습교육이 효과적인 교수-학습방법 임을 확인하였다.

후속연구

그 결과 시뮬레이션 모듈의 설계와 교육적인 측면에서 긍정적인 평가가 있었고, 학습자의 학습에 대한 만족도와 자신감 점수가 높았으므로 본 연구에서 개발한 시뮬레이션 모듈은 지역사회간호학 방문간호 실습교육 중 비위관 관리교육에 대한 돌봄제공자를 교육하는 역량을 강화하기에 적합하다고 할 수 있다. 그러나 본 연구는 단일군 사후 설계 연구로 연구결과를 해석하는데 한계가 있고, 관찰법을 통해 교육의 효과를 평가한 것이 아니고 학습자의 자기기입식 설문조사를 통해 평가하였으므로 연구결과 해석에 제한이 따른다.
따라서 본 연구자는 첫째, 본 연구에서 개발한 시뮬레이션 모듈의 교육적 가치가 확인되었으므로 지역사회간호학 실습 교육 중 의료기관 가정간호 또는 장기요양 방문간호 실습에서 활용할 것을 제언한다. 둘째, 시뮬레이션 교육 후 학생의 자신감과 만족도 뿐 만 아니라 시뮬레이션 교육의 근거와 장기적인 교육의 효과를 제시할 수 있도록 자기효능감, 임상적 판단능력과 수행능력 등의 변화에 대한 지속적이고 연계성 있는 연구를 제언한다. 셋째, 본 연구에서 개발한 시뮬레이션 모듈의 틀을 기반으로 지역사회간호의 다양한 영역에서 시뮬레이션 모듈을 개발하고 검증할 것을 제언한다.
