본 연구는 간호 대학생의 의사소통능력, 우울과 임상실습 스트레스를 파악하고, 임상실습 스트레스와의 관련요인을 분석하고자 시도되었다. 대상자는 B시에 소재한 간호 대학생 208명이며, 자료수집기간은 2017년 9월 1일부터 2017년 9월 30일까지였다. 수집된 자료는 SPSS WIN 21을 이용하여 기술통계, t-tset, ANOVA, Pearson's correlation, Stepwise multiple regression으로 분석하였다. 연구결과, 간호 대학생이 경험하는 의사소통능력은 5점 만점의 평균평점 3.74점이며, 우울은 4점 만점의 평균평점이 2.36점이며, 임상실습 스트레스는 5점 만점의 평균평점 2.88점으로 나타났다. 대상자의 제 특성에 따른 임상실습 스트레스 정도는 성별 (p=.002), 나이 (p=.048), 학년 (p=.001), 대학만족도 (p<.001), 전공만족도 (p=.002), 희망진로(p=.002), 실습만족도 (p<.001)에서 통계적으로 유의한 차이가 있었다. 대상자의 의사소통능력과 임상실습 스트레스는 역 상관관계를 보였고 (r=-.17, p=.016), 우울과 임상실습 스트레스는 상관관계를 보였으며 (r=.37, p<.001), 임상실습 스트레스에 유의한 영향을 주는 요인은 우울, 실습 만족도, 학년, 성별이었고, 전체 26.9%의 설명력을 갖는 것으로 나타났다. 그 결과, 간호 대학생의 임상 실습스트레스를 최소화하기 위하여 우울을 예방하고 완화하며 의사소통능력을 향상시킬 수 있는 중재 프로그램 개발이 필요하다고 사료된다.

This study was conducted to investigate the relationship between depression and communication skills and clinical practice stress among college nursing students. This study targeted 208 nursing college seniors in B city who have experience in clinical practice. Data were collected from September 1-30 2017 and were analyzed using descriptive analyses, t-tests, ANOVA, Pearson's correlation, and Stepwise multiple regression with the SPSS WIN 21.0 program. The average score of communication skills was 3.73, the score of depression was 2.36, and the score of clinical practice stress was 2.88. Significant differences were found in clinical practice stress depending on gender (p=0.002), age (p=0.048), grade (p=0.001), satisfaction with college life (p<0.001), satisfaction with major (p=0.002), desired course (p=0.002), and satisfaction with clinical practice (p<0.001). Clinical practice stress was negatively related to communication skills (r=-0.17, p=0.016) and positively related to depression (r=0.37, p<0.001). Depression, satisfaction with clinical practice, grade, sex and religion had a high explanation rate of 26.9%, indicating that they were meaningful variables that influenced clinical practice stress. Overall, to decrease clinical practice stress among nursing students, it is necessary to develop an intervention program that can prevent and mitigate depression and improve communication skills.

이론/모형

우울 측정도구는 Radloff(1977)가 제작한 CES-D(Center for Epidemiological Studies-Depression)을 Chon 과 Lee[21]가 한국판으로 번역한 도구로 측정하였다. ; 본 측정도구는 총 20개의 문항으로 긍정문항 4개, 부정문항 16개 문항으로 구성되어 있다. Likert 5점 척도로 점수가 높을수록 우울 정도가 높음을 의미한다.
의사소통능력 측정도구는 Rubin, Palmgreen과 Sypher[19]가 개발한 ‘Communication Research Measures’를 Hur[20]가 번역 및 수정 보완한 도구로 측정하였다. 본 측정도구는 총 15개 문항으로 15개의 핵심 하부차원(자기노출, 역지사지, 사회적 긴장완화, 주장적, 집중력, 상호작용관리, 표현력, 지지력, 즉시성, 효율성, 사회적 적절성, 조리성, 목표간파, 반응력, 잡음 통제력)을 반영하도록 구성되어 있다.
임상실습 스트레스 측정도구는 Beck Srivastsva[22]가 개발하였고 Kim 과 Lee[19]이 수정ㆍ보완한 임상실습 스트레스 도구로 측정하였다. 이 도구는 실습교육환경 5문항, 바람직하지 못한 역할모델 6문항, 실습업무 부담 4문항, 대인관계 갈등 4문항, 환자와의 갈등 5문항총 24문항으로 구성되어 있다.

성능/효과

본 연구는 간호대학생의 의사소통 능력, 우울이 임상실습 스트레스에 미치는 영향을 규명함으로써 간호대학생의 임상실습 스트레스를 감소시키는 전략을 개발하는데 기초자료를 제공하고자 실시하였다. 그 결과, 대상자의 제 특성에 따른 임상실습 스트레스 정도는 성별, 나이, 학년, 대학 생활 만족도, 전공 만족도, 진로, 실습 만족도에서 통계적으로 유의한 차이가 있었다. 대상자의 의사소통 능력과 임상실습 스트레스는 유의한 음의 상관 관계가 있었고, 우울과 임상실습 스트레스의 관계는 유의한 양의 상관관계가 있는 것으로 나타났다.

첫째, 의사소통능력, 우울, 임상실습스트레스 규명된 선행연구에서의 변수들을 포함시켜 통합적 관계의 구조 모형을 구축하여 검증하는 후속연구가 필요하다.

	임상실습 스트레스는 간호대학생들에게 어떤 영향을 미치는가?	이 뿐 아니라 대상자, 보호자, 의료진 및 병원 관계자와 대인관계에 있어 어려움을 많이 경험하며 역할수행 중무능감, 임상실무 및 실습과제의 과중함, 실습평가의 압박에 이르기까지 다양한 임상실습 스트레스를 경험하고 있다[4]. 이러한 임상실습 스트레스는 간호대학생들의 임상실습에 대한 흥미와 요구도를 낮출 뿐 아니라 비판적 사고력 및 문제해결능력에도 부정적 영향을 미치는 것으로 알려져있다[5]. 임상실습 스트레스가 해소되지 않을 때 간호대학생들은 실습이 두려워지고 간호에 대한 자신감을 잃게 되고, 간호학 전공에 대한 회의마저도 느끼게 되며 심각한 위축과 좌절을 경험한다[6].
