본 연구의 목적은 치매대상자를 돌보는 간호사의 공감역량과 소진이 의사소통 행위에 미치는 영향을 파악하는 것이다. 연구 설계는 서술적 조사연구를 사용하였다. 연구대상은 B시, U시 및 Y시에 소재하고 있는 9개 요양병원에서 직접 치매 대상자를 돌보는 간호사 190명이다. 자료수집기간은 2018년 6월 20일에서 7월 1일까지였으며, 자기보고식 설문지로 조사하였다. 수집된 자료는 SPSS 22.0 프로그램을 이용하여, 서술통계, t-test, ANOVA, pearson correlation coefficient, multiple regression으로 분석하였다. 본 연구결과에서 치매 대상자를 돌보는 간호사의 의사소통 행위는 공감역량 및 소진과 통계적으로 유의한 상관관계가 있는 것으로 나타났다. 다중회귀분석 결과, 치매 대상자를 돌보는 간호사의 공감역량과 소진은 의사소통 행위에 통계적으로 유의한 영향을 미치는 것으로 나타났으며, 이들 변수의 총 설명력은 31%이었다. 따라서, 추후 본 연구결과를 토대로 치매 대상자를 돌보는 간호사의 의사소통행위를 증진시키기 위해 공감역량을 증진시키고 소진을 감소시킬 수 있는 교육프로그램의 개발과 평가를 위한 지속적인 연구를 제언한다.

The purpose of this study was to investigate the influence of nurse's compassionate competence and burnout on their communication behavior with dementia patients. The subjects of this study were 190 nurses who were taking care of patients with dementia at long-term care hospitals and who agreed with the survey. The collected data were analyzed by descriptive statistics, t-test, ANOVA, Pearson Correlation Coefficient, and multiple regression. The communication behavior of nurses caring for dementia patients was found to have a significant correlation with compassionate competence and burnout. Multiple regression analysis showed that nurses' compassionate competence and burnout affected communication behavior, and the total explanatory power was 31%. It is suggested that the results of this study can be used as basic data for developing programs to improve communication behavior with patients with dementia.

본 연구의 대상자는 B시, U시와 Y시에 있는 9개 요양병원 간호사를 편의표출 하였으며, 구체적인 선정기준은 1) 요양병원에 근무하면서 2) 치매대상자를 돌보고 있으며 3) 질문지의 내용을 이해하고 답할 수 있는 자로 본 연구의 목적과 방법 등에 대해 설명을 듣고 자발적으로 연구 참여에 서면동의 한 자이다. 구체적인 제외기준은 1) 근무한지 3개월 미만이며 2) Part time으로 근무하고 있는 간호사이다.
자료수집 기간은 2018년 6월 20일에서 7월 1일까지였다. 총 205부의 설문지를 배부하고 192부를 회수하였으며(회수율 93.7%) 이중 불성실하게 작성된 설문지 2부를 제외하고 총 190부를 최종 분석하였다.

이론/모형

본 연구에서 간호사의 공감역량을 측정하기 위하여 Lee & Seomun[9]가 개발한 간호사의 공감역량 측정도구(Compassion Competence Scale: CCS)를 사용하였다. 이 도구는 소통력, 민감성, 통찰력 3개의 요인, 총 17개 항목으로 구성되어 있다.
본 연구에서 치매대상자와의 의사소통행위는 Lee [5] 가 개발한 치매대상자를 돌보는 간호사의 의사소통행위측정도구(Communication behavior scale of nurses caring for people with Dementia: CBS-D)를 사용하였다. 이 도구는 대화반응관리, 관계조절, 정서표현, 이해증진 4요인 18문항으로 구성되어 있으며 ‘전혀 그렇지 않다’ 1점에서 ‘항상 그렇다’ 5점의 Likert 척도로 측정되며 도구의 신뢰도는 Lee[5]의 연구에서 Cronbach’ s α.
소진은 Pines 등[22]이 개발하고 Peek [23]이 번역한 도구를 이용해 측정하였다. 이 도구는 신체적, 정서적,정신적 소진 3개 영역에 20개 문항으로 구성되어 있으며 ‘없다’ 1점 부터 ‘항상’ 5점의 likert 척도로 측정된다.

성능/효과

연구결과, 치매대상자를 돌보는 간호사의 의사소통행위는 간호사의 공감역량 및 소진과 유의한 상관관계를 가지고 있었다. 또한 요양병원 간호사의 공감역량과 소진은 치매대상자를 돌보는 간호사의 의사소통행위에 영향을 미치는 요인으로 확인되었다. 본 연구결과를 토대로 다음의 내용을 제언하고자 한다.
본 연구에서는 노인인구의 증가와 더불어 급증하고 있는 요양병원 간호사의 공감과 소진이 치매대상자와의 의사소통행위에 미치는 영향을 파악하였다. 연구결과, 치매대상자를 돌보는 간호사의 의사소통행위는 간호사의 공감역량 및 소진과 유의한 상관관계를 가지고 있었다. 또한 요양병원 간호사의 공감역량과 소진은 치매대상자를 돌보는 간호사의 의사소통행위에 영향을 미치는 요인으로 확인되었다.

후속연구

첫째, 치매 의사소통 교육유무에 따라 간호사의 의사소통행위 점수가 유의미하게 차이가 난 점에 주목하여 간호사를 대상으로 지속적인 치매 의사소통 교육이 요구된다. 둘째, 치매대상자를 돌보는 간호사의 의사소통행위를 증진시키기 위해 간호사의 공감역량을 증진시키고 소진을 감소시킬 수 있는 프로그램의 개발과 평가를 위한 지속적인 연구를 제언한다. 셋째, 다양한 실무현장에서 치매대상자를 돌보고 있는 간호사의 의사소통행위에 영향을 미치는 요인을 파악하는 반복연구를 제안한다.
둘째, 치매대상자를 돌보는 간호사의 의사소통행위를 증진시키기 위해 간호사의 공감역량을 증진시키고 소진을 감소시킬 수 있는 프로그램의 개발과 평가를 위한 지속적인 연구를 제언한다. 셋째, 다양한 실무현장에서 치매대상자를 돌보고 있는 간호사의 의사소통행위에 영향을 미치는 요인을 파악하는 반복연구를 제안한다.
간호연구 측면에서는 본 연구를 통하여 치매대상자를 돌보는 간호사의 의사소통행위에 영향을 미치는 요인을 확인할 수 있었다. 향후 연구결과에 근거하여 간호사의 의사소통역량강화 프로그램 개발 연구, 치매대상자를 돌보는 간호사의 의사소통행위에 영향을 미치는 다양한 변수를 확인하는 연구, 치매대상자를 돌보는 여러 직종 간의 의사소통행위 영향요인 비교연구, 요양병원 외에 다양한 임상실무 현장에서 치매대상자를 돌보는 간호사들 간의 영향요인 비교 연구가 이루어질 수 있을 것이다.

	치매대상자에게 공감적 자세를 취하는 것은 어떤 역할을 하는가?	공감역량을 지닌 간호사가 대상자의 감정을 정확하게 지각하여 이해하게 되면 대상자는 자신이 혼자가 아니라는 느낌과 자신이 이해받고 있다는 느낌을 갖게 되어 스스로를 가치 있게 받아들이게 된다[10]. 특히, 의사소통에 어려움을 겪는 치매대상자에게 공감적 자세를 취하는 것은 의미 있는 대화의 기반을 확립하고 공통된 이해와 평등성에 기초한 상호관계를 구축하는데 도움을 주어 의사소통을 증진하는 역할을 한다[11].
