Falls were also associated with increased length of hospital stay to 21.58 days (p<.001), and healthcare costs of 17,052,784 Won (p<.001). Conclusion: These findings can be a resource for fall prevention education and to help develop fall risk assessment tools for adults admitted to hematology units.

대상 데이터
본 연구에 사용된 연구자료는 자동화된 낙상위험사정시스템 구축을 위해 수집된 자료이며[14], 2009년 10월부터 2015년 6월까지 서울 소재 일개 상급종합대학병원에 입원하였다가 퇴원한 환자의 전자의무기록에서 추출되었다. 낙상 환자군은“의도하지 않았으나, 신체가 갑자기 조절되지 않아 바닥이나 아래쪽으로 이동한 경우”로 적정 진료팀에 보고된 환자들이며, 낙상이 발생할 뻔한 근접 오류는 제외하였다.
여러 번 입원한 경우 마지막 입원을, 동일 입원 기간중 낙상이 2회 이상 발생한 경우 마지막 낙상을 기준으로 추출된 자료이다. 원 자료에 포함된 변수는 1,145개(환자정보 130개, 간호기록 251개, 진단검사 89개, 감염 관련 6개, 수술 및 마취 82개, 간호기록 251개, 안전사고 43개, 수술분류 46개)이다.
본 연구는 원 자료에서 2013년 1월 1일부터 2014년 12월 31일까지 2년간 혈액내과 병동에 입원한 후 퇴원한 환자 318명(낙상 환자군 117명, 비낙상 환자군 201명)을 연구대상자로 선정하였다.

연구 변수는 의료의 질 개선에 주로 활용되는 Donabedian의 구조-과정-결과(Structure-Process-Outcome, SPO) 모델[15]을 기반으로 선정하였다. SPO 모델에서 구조적 요인은 환자 요인이나 환경적 요인을 포함하며, 과정적 요인은 치료나 간호중재와 관련된 요인을 포함한다.

이와 같이 선행연구에서 백혈구 수가 낙상 위험요인으로 규명되지 않아 직접적인 비교하기에는 곤란하지만,골수억제를 유발하는 항암화학요법이나 골수이식 등의 질병 치료 과정으로 인한 백혈구 감소증이 혈액내과 환자의 낙상에 간접적인 영향을 미친 것으로 판단된다. 따라서 백혈구 수가 감소한 환자에 대한 낙상 위험 경고와 낙상 예방교육을 시행할 필요가 있다고 판단되며, 백혈구 감소를 일으키는 다른 요인이 낙상 발생 요인일 가능성이 있을 수 있으므로 이와 관련된 반복연구를 제언한다.
이로 인해 감소된 근육 기능이 보행능력에 영향을 미쳐 일상 수행능력을 떨어뜨리기 때문에 낙상이 발생하는 것으로 생각된다. 따라서 혈중 알부민 수준의 변화를 관찰하여 낮은 수준의 알부민 수준을 교정하는 적극적인 중재요법을 시행하는 것이 낙상의 발생을 감소시키는 데에 도움이 되리라 생각된다.
마약성 진통제의 부작용인 진정, 어지럼증과 졸림이 낙상 위험을 높인 것으로 판단되므로 마약성 진통제를 복용하는 경우 낙상 위험에 대한 교육을 시행하고 낙상 예방 중재 활동을 시행할 필요성이 있다. 하지만 김은경 등[19]의 연구에서는 마약성 진통제가 낙상 발생에 영향을 미치는 요인으로 제시되지 않는 등 수행 연구 간에도 마약성 진통제가 낙상 위험요인인지 여부가 상반되어 나타나므로 추후 마약성 진통제에 대한 반복 연구의 필요성이 제기된다.
또한 본 연구에서는 대상자가 중환자실에 입실한 경우는 다루지 않았으며 환자 결과의 단기적인 측면만 파악한 것이라 부족한 면이 있다. 추후에는 혈액내과 환자의 낙상과 관련된 결과에 영향을 줄 수 있는 다른 변수를 추가적으로 보완해 장기적인 결과를 추적할 수 있는 연구를 제언한다.
