Purpose: We explored emergency nurses' experiences of coping with moral distress. Methods: A qualitative research design was used. We conducted in-depth interviews with 11 nurses working at a regional emergency medical center in South Korea. Data were analyzed using conventional content analysis. Results: The main theme of nurses' coping with moral distress was "a passive emotion-based response." We also extracted three categories of coping strategies: "uncritical adaptation to anguished situation," "self-justification for not acting," and "flight from ethical decision making." Conclusion: Nurses coped with moral distress in a passive and emotional way, possibly because of perceived incompetence and compliance to an organizational hierarchy. The findings imply that effective approaches to addressing moral distress in emergency nurses are needed at the individual and organizational levels.

제안 방법

연구 질문은 반구조화된 질문으로 도덕적 고뇌의 상황, 인지, 정서, 행동적 측면을 모두 포함하고 그에 대한 대응 전체를 포괄하기 위한 구체적인 내용으로 구성하였다. 면담의 주요 질문 내용은 응급실에서 근무하면서 도덕적 고뇌를 경험한 상황과 그에 대한 대응이 어떠했는지, 그리고 도덕적 고뇌에 효과적으로 대응하는데 도움이 된 방법이 무엇이었는지 등을 포함하여 구성하였다. 원활한 면담을 위해 면담 시작 시에 참여자에게 Jameton (1993)의 정의에 따른 도덕적 고뇌가 무엇인지에 대해 설명하여 이해를 도운 후 심층 면담을 진행하였다.
참여자의 면담은 2015년 2월 한 달간 자료의 포화까지 실시되었다. 자료의 포화시점은 동일한 답변이 2회 이상 반복해서 나오는 때로 보았다.

대상 데이터

연구의 참여자는 응급실에서 도덕적 고뇌를 경험하고, 이에 대응한 응급실 경력간호사 11명을 대상으로 하였다. 목적적 표집법에 따라 K지역에 위치한 종합병원 권역응급센터에서 1년 이상 근무한 간호사 11명이 참여하였다. 면담 자료 분석 과정에서, 자료의 포화가 이루어져 추가 모집이 필요하지 않으므로 11명의 인터뷰 인원이 적절한 것으로 판단하였다.

데이터처리

본 연구에서는 질적 내용 분석의 방법 중 전통적 내용 분석을 사용하여 탐구하였다. 이러한 질적 내용 분석 방법은 기존에 알려진 선입견을 배제하고 사람과 사건을 이해하려고 노력하는 탐구적인 방법으로 특히 전통적 자료 분석 방법은 기존의 연구와는 다른 차별적 인식을 새롭게 제공할 수 있다는 장점이 있다(Hsieh & Shannon, 2005).

성능/효과

• 도덕적 고뇌 문제를 해결하지 못한 과거 경험

참여자들은 과거의 도덕적 고뇌 상황에서 문제 해결을 시도하였을 때 실패했던 경험으로 인해 다시 당면한 도덕적 고뇌의 상황에서 윤리적인 결정에 따라 주체적인 행동을 하는 것에 대해 유보 하는 태도를 보였다. 관련 진술 내용은 다음과 같다.
연구 결과, 참여자들은 도덕적 고뇌에 대해 감정 중심의 수동 대응을 하고 있었다. 이는 윤리 문제에 당면한 간호사 개인의 내적 보호가 하나의 원인으로 작용하였다.
본 연구결과 응급실 간호사들의 도덕적 고뇌에 대응의 주제는 감정중심의 수동 대응이었다. 이에 따른 범주는 ‘관계를 고려한 당면한 상황 수용’, ‘윤리적인 행동을 하지 않음에 대한 자기 합리화’, ‘도피-회피’였다. 이러한 결과는 기능적 역할을 강조하는 간호 조직의 분위기 내에서, 역할 이행과 상충되는 상황의 도덕적 역할을 요구받는 간호사가 당면한 문제에 대해 드러내는 하나의 내적 보호의 대응 방식으로 볼 수 있다(Bartram et al.

후속연구

첫째, 응급실에서의 도덕적 고뇌에 대한 건설적인 대응을 지원하는 상황의 특수성을 고려한 맞춤형 프로그램 개발이 필요하다. 둘째, 실무 부서에 맞춘 도덕적 고뇌의 경험적 증거와 해결책을 토론하고 이를 누적하여 공유할 수 있는 개방적인 간호조직문화의 활성화가 필요하다.
본 연구 결과를 바탕으로 다음과 같은 제언을 하고자 한다. 첫째, 응급실에서의 도덕적 고뇌에 대한 건설적인 대응을 지원하는 상황의 특수성을 고려한 맞춤형 프로그램 개발이 필요하다. 둘째, 실무 부서에 맞춘 도덕적 고뇌의 경험적 증거와 해결책을 토론하고 이를 누적하여 공유할 수 있는 개방적인 간호조직문화의 활성화가 필요하다.
