Objectives: This study examined factors influencing hospital workers' perception of patient safety culture, job boredom and job crafting in reporting of safety compliance. Methods: Structured questionnaires were used to collect data from 245 hospital workers who were full-time staff for six months or more. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and hierarchical multiple regressions with SPSS/WIN version 22.0. Results: The mean score for perception of patient safety culture was 3.16±0.32, for job boredom was 2.52±0.52, for job crafting was 3.35±0.49 and for degree of safety compliance was 2.87±0.3. The hierarchical multiple analysis revealed that hospital work environment (β=.27, p<.001), communication process (β=.27, p<.001) of subarea perception of patient safety culture and cognitive crafting (β=.15, p=.039) of subarea job crafting were predictive of patient safety compliance (R2=.47, p<.001). Conclusions: The findings indicate that perception of patient safety culture and job crafting are important factors to be considered in working to improve hospital workers' safety compliance. Therefore, education and systematic programs to improve perception of patient safety culture and job crafting should be developed.

대상자의 안전이행에 영향을 미치는 요인은 위계적 회귀분석(Hierarchical multiple regression analysis)으로 분석하였다.
대상자의 일반적 특성에 따른 안전이행의 차이는 Independent t-test, one-way ANOVA, 사후검정은 Scheffé를 이용하여 분석하였다.
대상자의 환자안전문화에 대한 인식, 직무권태, 잡 크래프팅, 안전이행 간의 상관관계는 Pear-son’s correlation coefficients로 분석하였다.

이론/모형

안전이행은 직무안전을 유지하기 위하여 개개인에 의해 수행되도록 요구되는 주요 안전활동으로 의료기관의 안전관련 정책이나 지침을 따르는 행위를 의미한다[17]. 본 연구에서 안전이행은Hayers 등[19]이 개발하고 Chung [20]이 번안하여 사용한 도구를 승인을 받아 사용하였다. 도구의 문항 구성은 ‘나는 내가 처한 상황에 관계없이 모든 안전지침을 따른다.
잡 크래프팅(Job crafting)이란 조직구성원 개인의 욕구, 능력, 선호도와 직무특성 간의 적합도를 높이기 위해 주도적으로 자신의 직무를 만들어 나가는 것을 의미한다[13]. 본 연구에서 잡 크래프팅은 Slemp & Vella-Brodrick [18]이 개발한 Job Crafting Questionnaire(JCQ)를 Lim et al.[13]이 수정, 보완한 도구를 개발자의 승인을 받아 사용하였다. 이 도구는 업무 크래프팅 5개 문항, 인지 크래프팅 5개 문항, 관계 크래프팅 5게 문항으로 구성되었고, 문항 구성은 업무 크래프팅의 ‘나는 일을 더 잘하기 위하여 새로운 방식을 시도해 본다.
직무권태는 일을 수행하는데 있어 일이 하기 싫고 의욕이 나지 않으며 무기력감을 느낄 때를 의미한다[10]. 본 연구에서는 Lee [16]가 개발한 직무 권태 척도 중 일부 문항을 번안·수정하여 사용한Seong [11]의 측정도구를 승인을 받아 사용하였다. 이 도구는 총 10개 문항으로 구성되었고, Likert 5점 척도로 측정되었으며, 점수가 높을수록 직무권태가 높음을 의미한다.
환자안전문화에 대한 인식은 미국보건복지부산하기관인 의료관리조사품질국(Agency for Health care Research and Quality, 이하 AHRQ)에서 개발한 Hospital Survey of Patient Safety Culture를 Kim et al.[16]이 수정ㆍ보완한 도구를 승인을 받은 후 사용하였다. AHRQ 설문지는 부서의 근무환경 18개 문항, 환자안전과 관련된 직속상관이나 관리자의 태도에 관한 4개 문항, 환자안전문제에 관련된 의사소통과 절차에 대한 6개 문항, 근무부서에서의 사고발생시 보고의 빈도에 관한 3개 문항, 환자안전과 관련된 병원환경에 관한 11개 문항, 환자의 안전과 관련해서 근무하는 부서에 대한 전반적인 평가를 묻는 1개 문항을 합하여 총 43개 문항으로 구성되어 있다.

성능/효과

본 연구 제 변수들 간의 상관관계를 분석한 결과, 모든 변수들 간의 유의한 상관관계가 있는 것으로 분석되었다(p=<.001). 환자안전문화에 대한 인식은 직무권태(r=-.
본 연구는 일개 종합병원종사자의 환자안전문화에 대한 인식, 직무권태, 잡 크래프팅, 안전이행 정도를 분석하였고, 환자안전문화에 대한 인식 정도는 성별, 연령, 직종, 총 병원근무경력에 따라 직무 권태는 성별, 연령, 총 병원근무경력, 현 근무지경력에 따라, 잡 크래프팅은 성별, 연령에 따라 안전이행 정도는 성별에 따라 차이가 있음을 검증하였다. 안전이행의 정도는 남성이 여성에 비해 높게 나타났고, 안전이행 정도에 영향을 미치는 요인으로 환자안전문화에 대한 인식의 하부영역 중 병원 근무환경, 의사소통 절차와 잡 크래프팅의 하부영역인 인지 크래프팅 등이었고 안전이행의 47.
이는 대상자와 직무에 따른 차이로 생각되며, 의료종사자의 경우 변화하는 의료 환경에 적응하고, 업무강도가 증가되고, 다양한 평가를 준비하고 수행하는 과정을 경험하면서[22] 타 직종보다 직무권태가 낮은 것으로 생각된다. 본 연구에서는 직무권태가 안전이행 영향 요인이 아니며, 직종에 따른 차이도 없는 것으로 나타났다. 그러나 직무권태는 일에 대한 관심이나, 신중성, 적극성, 몰입 등이 낮아지고, 사고율 증가와도 관련이 있으므로[10], 이에 대한 정책적 지지는 필요하다고 생각된다.

후속연구

본 연구의 제한점은 일개 종합병원에 근무하는 의료종사자들을 대상으로 하였고, 간호직종이 절반 이상을 차지하고 있어 일반화하는 것에 대해서는 신중을 기해야 한다. 의료종사자들을 대상으로 한 안전이행에 대한 선행연구가 부족하여 비교, 검토에는 한계가 있었다.
추후 의료종사자들의 직종 간 대표성을 나타낼 수 있도록 표본 수를 늘여 비교분석하는 연구가 필요하고, 종합병원종사자의 안전이행에 대한 직종별 전략과 관리를 효율적으로 구축할 수 있을 것이다.
