Purpose: This study aims to identify the present handover status and the difference in performance and importance of handover between 119 paramedics and emergency department (ED) staff to improve the quality of handover. Methods: The study involved a questionnaire survey of 171 paramedics and 108 ED staff in four regions from August 28 to September 23, 2020. Subsequently, 279 questionnaires were collected and 277 copies were used for this study. Data were analyzed by SPSS 26.0 version. Results: The results showed that most of the paramedics and the ED staff rely on memory and verbally handover, without formal education about the handover process. And there were significant differences in performance in important information of prehospital. Some of the handover items were a difference in performance and importance between the paramedics and the ED staff. Conclusion: The unified education based on required and optional items is necessary to reduce the loss of information between the paramedics and ED staff, and standardized tools need to be developed in the handover process.

제안 방법

또한 119구급대원과 응급실 의료진 각각 그룹에서 인수인계 항목에 대한 수행도와 중요도 간의 차이를 확인하기 위하여 중요도-수행도 분석(importance -performance analysis, 이하 IPA) 기법을 활용하였으며, 수행도와 중요도의 평균값으로 4사분면(유지강화영역, 집중개선영역, 낮은 우선순위영역, 과잉노력지양영역)을 구분하여 2×2매트릭스를 만들었다.

대상 데이터

본 연구에 사용한 설문지는 119구급대원용과 응급실 의료진용으로 각각 구성하였다.

데이터처리

인수인계 각 항목에 대한 119구급대원과 응급실 의료진 간의 수행도와 중요도의 차이는 5점 척도에서 5, 4점을 그렇다로, 3점을 중립, 1, 2점을 그렇지 않다로 묶어 chi-square test 및 Fisher’s exact test를 실시하였다
0(IBM, USA)을 이용하여 분석을 실시하였다. 일반적 분석에서는 빈도와 백분율, 평균과 표준편차를 구하였다. 인수인계 각 항목에 대한 119구급대원과 응급실 의료진 간의 수행도와 중요도의 차이는 5점 척도에서 5, 4점을 그렇다로, 3점을 중립, 1, 2점을 그렇지 않다로 묶어 chi-square test 및 Fisher’s exact test를 실시하였다.

성능/효과

119구급대원(94.7%)과 응급실 의료진(99.0%) 모두 병원 전 단계에서 병원 단계로 이어지는 인수인계에 대해 중요하다고 인지하고 있었으며, 119구급대원의 73.5%와 응급실 의료진의 66.4%가 현재의 인수인계 과정에 개선이 필요하다고 응답하였다. 개선을 위한 방안으로 119구급대원은 ‘인수인계 태도 및 의사소통 개선(48.

인수인계 항목에 대한 중요도-수행도 분석(IPA) 매트릭스에서 119구급대원과 응급실 의료진은 전반적으로 중요도를 높게 인식하는 항목에 대해 높은 수행도를 보여 현재 인수인계 시 주요항목에 대한 중요성을 충분히 인지하고 적절하게 수행하고 있었다. 하지만 일부 항목에서 대상자 간의 수행도와 중요도의 차이가 있었다.
119구급대원과 응급실 의료진이 인수인계 항목에서 중요하다고 인식하며 높은 수행도를 보이는 11개의 항목에 대해서는 인수인계 필수항목으로, 중요도와 수행도를 모두 낮게 인식한 8개의 항목에 대해서는 인수인계 선택항목으로 구성하여 필요시 선택적으로 인수인계 할 수 있도록 구성해야 한다. 현재 119구급대원은 인수인계시 기억력에 의존하여(37.6%), 응급실 의료진은 구두로만 인계를 받아 기억(66.7%)하는 방식으로 주로 인수인계를 수행하고 있었으며, 119구급대원의 27.1%(46명), 응급실 의료진의 20.6%(22명)만 인수인계 교육 경험을 가지고 있어 민원, 중복인계, 업무지연 등의 오류를 인수인계 과정에서 경험하였다.

후속연구

개선을 위한 방안으로 119구급대원은 ‘인수인계 태도 및 의사소통 개선(48.8%)’, 응급실 의료진은 ‘인수인계 방식과 내용 개선(43.5%)’이 필요하다고 기술하여, 추후 인수인계 교육과정이 마련된다면 인수인계 도구를 사용한 고품질의 인수인계와 더불어 의사소통 자세 및 기본 규범에 대한 교육 내용이 함께 추가되어야 할 것이다.
‘보호자 동행 여부’에서 119구급대원에 비해 응급실 의료진의 중요도가 높게 나타났는데, 이는 119구급대원이 병원 이송과정에서 한 명의 환자에 대한 응급처치를 주로 담당하는 것에 반해 응급의료기관에서는 진료 시 접수가 필수적이고 다양한 환자들이 처치를 받고 있으며, 때에 따라서는 수술적 치료, 입원 등이 요구되어 보호자의 동의가 중요하기 때문일 것이다. 따라서 119구급대원은 환자가 응급실 내에서 진료를 받을 시 보호자 동행 여부가 중요함을 인식하고 응급실 의료진과의 인수인계시 해당 여부를 함께 인계하여야 할 것이다. ‘(소아)몸무게’는 응급실 의료진에 비해 119구급대원이 중요도를 높게 인식한 항목으로, 소아 환자에게 약물이나 응급처치 장비 등을 사용 시 의료기관에 비해 제한된 현장의 인력이 소아 환자의 몸무게를 고려하여 적용해야 하기 때문에 중요도를 높게 인식한 것으로 사료된다.
이와 같이 119구급대원과 응급실 의료진의 수행도와 중요도 차이가 관찰된 항목을 분석하고, 수행도와 중요도가 높게 나타난 11개의 항목에 보호자 동행 여부를 포함시켜 총 12개의 항목은 필수항목으로, 그 외에 수행도와 중요도가 낮게 나타난 항목은 선택항목으로 인수인계 표준항목을 구성한다면 바쁜 현장에서 효율적인 인수인계를 수행할 수 있을 뿐만 아니라 서로의 업무에 대한 인식 차이를 줄여나갈 수 있는 의미 있는 인수인계 도구가 개발될 것이다.
