본 연구개발의 목표는 첫째, 항생제의 생산과 유통, 소비 과정을 국가 규모에서 파악하며, 둘째, 생산된 항생제가 의료기관에서 소비되는 사용 양상과 그 관련요인을 분석하며, 셋째, 치료적 항생제 사용에 대한 적정성 평가와 새로운 지침을 개발하고 넷째, 예방적 항생제 사용에 대한 적정성 평가와 새로운 지침을 개발하며, 다섯째, 항생제의 적정 사용을 위한 국가 규모의 정책적인 방안을 제시하고, 여섯째, 병원내에서의 적절한 항생제 사용을 위한 항생제 사용규제 프로그램을 개발하는 데 있다. 이를 위하여 3년 간의 연구기간 동안 과제를 네 개의 세부과제로 나누어 다기관 공동연구를 시행하였다. 제 1 세부과제에서는 국내 항생제의 생산, 유통 및 소비구조에 대한 체계분석을 하고자 하였으며 제 2세부과제에서는 의료기관에서의 항생제 사용양상과 DRG저불제도 시범사업 실시 전후의 항생제 사용양상을 비교하였다. 제 3세부과제에서는 치료적, 수술의 예방적 항생제 사용의 적정성 평가 및 사용지침을 개발하고자 하였다. 마지막으로 제 4세부과제에서는 병원단위의 항생제 사용규제 프로그램의 개발 및 효과를 분석하였다.
항생제의 생산은 비교적 안정적인 규모를 유지하고 있었으나, 제약업체의 영세성과 연구개발비 투자의 부족으로 인한 신약개발 미흡, 이로 인한 재가공(copy) 약품의 과잉생산이 문제점으로 대두되고 있었다. 또한 항생제 유통과 소비에 관한 통계자료가 정부기관, 제약협회, 민간 의약정보 관리회사 등에서 체계적으로 분석, 관리되지 못하고 있으며, 특히 유통관련자료 통계논 각 제약회사나 관련조합들이 체계적으로 관리하지 않는 것으로 파악되었다. 따라서 항생제의 생산, 유통 및 소비의 양적 질적 모니터링을 위한 시스템이 필요하다. 그러나 본 과제 결과 현재 국내의 항생제의 질 관리와 항생제 모니터링 수준은 제한적이고 후진적이라고 판단된다. 그러므로 국가 수준의 항생제 surveillance 시스템 필요하며, 표본추출방법 등의 조사방법 개발 필요하고, EDI-based surveillance system구축이 필요하며, 유통정보센터에 대한 우려를 불식 필요가 있을 것이다. 의료기관에서 소비되는 항생제약 사용 양상과 그 관련요인에 대한 분석 결과를 보면 97년 전체 약제비중 항생제의 비율은 약 35% 로 가장 컸으며, 항생제 사용량을 보면 Cephalosporin이 전체 항생제 사용일수의 52.5%를 차지하고 있어 가장 많이 사용되고 있는 항생제였다. 3차 의료가관에서의 항생제 사용양상 중 가장 큰 문제점으로 나타난 것은 예방 목적의 항생제 사용이었다. 외과계에서의 전체 항생제 사용중 예방적 목적으로 사용된 것은 563예(91.9%)였으며, 적응증, 약제의 선택, 투여기간, 투여시기 등 거의 모든 기준에서 적절하지 못한 것으로 나타나고 있는 실정이었다. 적절한 적응증이 61.1%, 적절한 투여시기에 맞게 투여한 경우가 55% 이었다. 또한 적절한 투여기간 준수한 경우가 23.5% 이였다. 치료적 항생제 사용양상을 보면 적절한 적응증에 맞게 투여된 경우는 6 1.7% 이었다. 치료적 목적으로 사용된 항생제의 사용일수는 내과에서는 3세대 cephalosporin이 23.4%로 가장 높았다. 의료비 지불제도에 따른 항생제 사용양상의 변화를 보기 위해 DRG 시범사업 실시 전후의 항생제 사용량과 사용총액을 비교하였는데, 2차년도에 시행한 항생제 사용양상 변화조사에서는 대상병원의 대상질환군 마다 비용변화양상이 다르게 나타났다. 이에 3차년도에는 항생제 사용액, 사용비용 비교뿐만 아니라 입원중, 퇴원시, 외래에서의 항생제 사용량을 구분하고 경구/주사제의 비율을 조사하며 항생제 종류, 단가의 변화를 조사하였다. 그 결과 재원열수의 유의한 감소와 항생제 사용총액이 감소하는 양상을 보였다. 또한 일일 제한 사용량인 DDD (daily defined dose)로 환산한 항생제 총 사용량의 경우 유의하게 감소하는 것을 보였고, 사용 항생제의 평균가격 또한 유의하게 감소하였다. 치료적 항생제 사용에 대한 적정성 평가와 새로운 지침을 개발하고, 예방적 항생제 사용에 대한 적정성 평가와 새로운 지침을 개발하고자 하였던 3세부과제의 결과를 보면 호흡기계 감염증인 폐렴과 급성인두염의 경우 99.8%에서 항생제를 사용하고 있었으며, 감기의 경우 원칙상 대부분이 항생제를 사용할 필요가 없는데도 불구하고 97.2%에서 항생제를 처방하고 있었다. 발열성 과립구 감소증에 대한 경험적 항생제 사용 지침은 문서화된 원내 protocoI이 있는 병원은 4곳이었다. 과립구 감소증이 있을 경우 발열이 없더라도 예방적으로 항생제를 사용하는 병원이 2곳이 있었으며, 이때 사용하는 항생제는 quinolone제제와 TMP/SMX이라고 하였다. 경험적 항생제 치료의 초기 선택약제는 AntipseudomonaI 3rd cephalosporin(Ceftazidime, Cefoperazone/sulbactam) + Aminoglycoside이 가장 많았다. 호흡기 및 요로 감염의 흔한 원인 균주는 각각 S. pneumoniae와 E. coli가 가장 많았으며, 모두 높은 항생제 내성율을 보이고 있었다. 이상의 자료들을 기초하고 국내외 문헌들을 참고하여 각 질환들에 대한 항생제 사용 권장지침을 수립하였다. 사용 권장지침의 대상질환은 호흡기감염증(폐렴), 요로감염증, 발열성 호중구 감소증, 그리고 수술의 예방적 항생제이다. 수립된 각 질환에 대한 항생제 사용 지침은 국내 감염 관련 학회의 검증을 거쳐 권장지침으로 수립되었다. 병원내에서의 적절한 항생제 사용을 위한 항생제 사용규제 프로그램올 개발하기 위하여 진행되었던 4세부과제의 결과를 보면 19개 병원 중 18개 병원이 어떤 방법으로든 항생제 사용관리를 한다고 하였으며, 특정 항생제 사용제한(제한항균제 시스템)을 17개 병원이 운영하고 있었고, 감염내과에 자문 의뢰를 통하여 관리하는 병원이 15곳이었다. 항균제 사용관리가 시행되고 있는 병원단위에서 1년간 항생제 사용량과 임상 분리 주요 미생물의 항생제 내성율과의 상관관계를 보면 3세대 cepha1osporins AUD와 K. pneumoniae의 cefotaxime 내성율이 유의한 역상관관계를 나타낸 야외에는 별 관련성이 없었다. 이는 1년간의 단기간이 항생제 사용으로 인한 내성 발현이 가시적으로 나타나기에는 짧다는 것과 내성균의 증가에 항생제의 사용 요인이외에 내성균의 병원감염관리가 얼마나 효과적이었느냐는 변수가 있었기 때문으로 생각된다. 항균제 관리위원회의 조치 사항에 대한 영향을 보기 위해 제한 예정 항균제 공시후 사용량 (AUD) 및 보험 삭감액의변화를 관창하였는데, 삭감액이 감소한 것으로 나타나 항균제 관리위원회의 영향을 알 수 있었다.

The purpose of this project was first, to understand the nationwide production, dis-tribution and consumption of antibiotics second, to analyze the consumption pattern of the antibiotics and its related factors in hospitals third, assess the reasonable use of thera-peutic antibiotics and develop a new practice guideline fourth, assess the appropriateness of prophylactic antibiotic use fifth, to propose a national policy and plan for appropriate use of antibiotics sixth, development of restriction program for proper use of antibiotics in the hospitals. We divided this project into four different subjects and conducted a multicenter study for three years. In the first subject, we analyzed the production, distribution and consumption structure of antibiotics and in the second subject, we compared consumption pattern of antibiotics in the hospitals and consumption pattern before and after the execution of DRG payment system. In the third subject, we evaluated the reasonableness of therapeutic and preoperative use of antibiotics for prevention and developed a practice guideline. Finally, in the fourth subject we analyzed the developed restriction program for antibiotics and its effect.
Production of antibiotics has been relatively stable but due to small pharmaceutical manufacturing companies and small investment for research and development of new drugs, overproduction of copy drugs has become a new problem. Also, statistical data on distribution and consumption of antibiotics have not been systematically managed and analyzed by government agencies, association of pharmaceutical manufacturing companies and private medical-pharmaceutical information service companies. Especially statistical data on distribution of antibiotics were not managed systematically by phamlaceutical manufacturing companies and its associated unions. So, systems for monitoring production, distribution and consumption of antibiotics quantitatively and qualitatively are needed. Result of this project showed that current level of quality control and monitoring of antibiotics were limited and not well developed in this country. Therefore development of nationwide antibiotic surveillance system, sampling method and EDI-based surveillance system is needed. Result of the analyzed data about consumption pattern of antibiotics and its related factors showed that in 1997, antibiotics shared largest proportion(35%) in total amount of money dispensed in medication. Among antibiotics consumed, cephalosporin was the most commonly prescribed antibiotics(52.5%).
In large hospital, prophylactic use of antibiotics was the biggest problem. Surgical department used antibiotics for prophylaxis in 563 cases(91.9%) and result of most of the evaluated standard such as indication for antibiotics use, choice of antibiotics, administered time and duration were inadequate. Only 61.1% of cases used antibiotics adequately and administered time of antibiotics were adequate in 55% of cases. Duration of antibiotics use' was only adequate in 23.5%. In therapeutic aspect, antibiotics were used adequately in 61.7% and third generation cephalosporin was(23.4%) used most commonly in department of medicine. Consumption pattern and total amount of money spent on using antibiotics were analyzed and compared before and after execution of DRG payment system. At second year of this study, changing pattern of consumption of antibiotics was different in every hospital participating in this project and different in every disease. So, in the third year, we analyzed not only the total amount of money spent for antibiotics but also changing pattern of cost and class of antibiotics consumed and prescription ratio of oraIl intravenous antibiotics according to admission, discharge and out-patient clinic. As a result, decrease in total hospital stay and amount of money spent in antibiotics were noted. If we convert total amount of antibiotics used in the hospitals into DDDCdefined daily dose), significant decrease was noted. Average cost of antibiotics was decreased also.
If we look at the result of third subject, antibiotics were used in 99.8% of respiratory infection such as pneumonia and acute pharyngitis. Although it is not indicated in common cold, antibiotics were prescribed in 97.2%. Only four hospitals had their own guideline for empirical antibiotic therapy in febrile neutropenic patients. Two hospitals used prophylactic antibiotics such as quinolone and TMP/SMX for nonfebrile neutropenic patients. For initial empirical antibiotic therapy, antipseudomonal third generation cephalosporin CCeftazidime, Cefoperazone/sulbactam) and aminoglycoside were chosen most commonly. The common causative organisms of respiratory and urinary tract infection were S. pneumoniae and E. coli. each and both organism showed high rate of resistance to the antibiotics. Based on the data stated above and reported in domestic and foreign literatures we made a practice guideline for respiratory tract infection(pneumonia), urinary tract infection, febrile neutropenia and preoperative antibiotics prophylaxis. These practice guidelines were approved by academic societies related to infectious disease in our country.
Fourth subject was carried out to develop the control program for proper antibiotics use in the hospital. Eighteen of the nineteen hospitals participating in this subject were using. different kinds of program to control the antibiotic lise. Seventeen hospitals had their own restriction program on specific antibiotics and among them fifteen hospitals restricted dispense of specific antibiotics by requiring ratification by infectious disease specialist.
We evaluated correlation between the amount of antibiotics consumed in a year and antimicrobial resistance of major pathogen. Cephalosporins AUDCAntimicrobial Use Density) was inversely correlated with cefotaxime resistance rate of K. pneumoniae and the others show no significance. It could be argued that observation time of one year was too short to determine the effect of antibiotic use on antimicrobial resistance and efficacy of management program on resistant organism might impact on the results. We intend to look into the effect the antibiotics management committee on antibiotics amount(AUD) and insurance curtail bills. The curtail fees were reduced since the committee was organized and administered.
