파킨슨 질환의 치료에 최우선적으로 사용되고 있는 레보도파의 갑작스러운 혈중농도 저하로 인한 파킨슨 증상의 약화에 신속하고 편리하게 사용할 수 있는 응급용 비강 투여 제형을 개발하고자 하였다. 이 연구에서는 레보도파의 용해도 (25 ℃, 0.05 mol/L 인산염완충액)가 1.65 mg/mL로 낮은 점을 고려하여 이 약물의 가용화 연구를 수행하였으며, 물리화학적 안정성 및 각종 점막 추출액에서의 안정성과 수종의 항산화제의 영향을 평가하였고, 레보도파의 비강 등 점막 투과에 미치는 용제, pH 및 투과 촉진제류의 영향을 검토하였다. 이를 바탕으로 레보도파 함유 액상 조성물을 설계하고 이를 동결 건조하여 가루로 하였다. 동결건조분말의 함량과 보존 안정성을 검토한 다음 이를 재용해하여 비강 점막 투과 특성, 비강 점막 손상을 평가하고, 흰쥐를 실험동물로 하여 비강 및 경구 투여한 후 각 투여경로별로 혈중 및 뇌중 레보도파의 약물동태학적 파라미터를 측정하여 비교 검토하였다. 이상의 연구로로부터 얻은 주요 결과는 다음과 같다. 1. 레보도파의 가용화 수성 및 비수성 용제 중 레보도파의 용해도를 측정한 결과 물 (30 ℃)에서의 용해도는 3.90 mg/mL이었으며 이에 비하여 0.04 mol/L 인산에서의 용해도는 8.72 mg/mL로서 약 2.2 배 높았으며 pH 1.2 완충 수용액에서 레보도파의 용해도는 12.8 mg/mL로서 pH 3까지는 용해도가 낮아졌으며 pH 8까지는 거의 일정하였다. 난용성 약물의 가용화에 널리 쓰여 온 CD류, 담즙산염인 SDC 및 물 감응성의 점막 부착성 고분자인 Gantrez 139?? 등의 가용화 효과는 현저하지 않았으며 PEG, PG, IPA 및 Transcutol?? 등 유기용제는 친수성인 레보도파를 거의 녹이지 못하였다. 여러 종류의 유기산 용액 중에서 레보도파의 용해도 크기는 0.1 mol/L 농도에서 메탄설폰산 > 말레산 > 시트르산 > 타르타르산 > 말산의 순서이었으며 이는 물에서의 용해도에 비하여 각각 6.1, 3.1, 1.7, 1.5 및 1.4 배 증가시킨 결과이다. 메탄설폰산은 몰 농도 증가에 따라 레보도파의 용해도를 농도 의존적으로 급격하게 향상시켰으나 1 mol/L 메탄설폰산은 그 pH가 -0.41로서 매우 강산성을 나타내어 액상제제의 첨가제로서는 바람직하지 않았다. 0.1 mol/L 말레산용액 (pH 2.0) 중에서 레보도파의 용해도는 12.11 mg/mL이었으며 말레산의 몰 농도가 0.3, 0.5, 1, 1.5 및 2 mol/L로 증가함에 따라 농도 의존적으로 용해도가 향상되었다. 또한 pH 2.2 완충액 중에서 용해도가 6.6 mg/mL이었던 것과 비교하면 같은 pH일지라도 인산염완충액보다는 말레산용액 중에서 용해도가 높게 나타났다. 이로 볼 때 레보도파는 산성이 클수록 용해도가 증대하고 완충종도 용해도에 영향을 미침을 알 수 있다. 2. 레보도파의 수용액 및 점막 추출액 중 안정성 레보도파의 안정성은 수용액의 pH와 온도에 의하여 크게 영향을 받는 것으로 나타났다. 수용액에서 레보도파는 산성이 강할수록 높은 잔존률을 보인 반면 중성 및 약알칼리성으로 갈수록 급속한 분해거동을 나타내었으며, 에데트산나트륨, 아황산수소나트륨 및 아스코르브산과 같은 항산화제류는 레보도파의 안정화에 별다른 효과가 없었다. 점막 추출액 중 레보도파의 효소적 안정성은 비강, 십이지장 및 직장 점막 추출액 중에서 레보도파의 초기 농도가 20 μg/mL인 경우 5분까지 약 88 %의 잔존률을 보인 반면 100 μg/mL의 높은 농도에서는 1 시간까지 약 98 % 이상을 나타내어 레보도파의 분해에는 농도에 따른 분해효소의 포화 현상이 존재하는 것으로 생각된다. 그러나 레보도파의 낮은 농도에서도 각종 점막 추출액 중에서 탈탄산 효소 억제제인 카르비도파의 첨가 효과가 나타나지 않은 것은 점막 추출액 중 탈탄산 효소 이외에 다른 분해효소가 관여되었을 것으로 생각된다. 3. 레보도파의 비강 점막 투과 특성 점막 부위별로 레보도파용액(2 mg/mL) 100 μL를 donor cell에 넣어 투과시험을 한 결과 비강 점막의 투과 flux는 145.4 μg/㎠/hr로서 십이지장(19.8 μg/㎠/hr) 및 직장(3.9 μg/㎠/hr)에 비하여 각각 7 및 37 배 높았다. 레보도파의 donor 농도가 0.1, 1, 2, 및 4 mg/mL로 증가할수록 레보도파의 투과 flux는 20.1, 33.9, 102,9 및 148.6 μg/㎠/hr으로 농도 의존적인 투과 (y = 0.3052 x + 0.0461, R2 = 0.9924)를 나타내었다. 이는 레보도파의 비강 흡수를 증가시키기 위해서는 투여용액 중 레보도파의 농도를 최대한 증대시켜야 함을 의미한다. 레보도파의 비강 점막 투과에 미치는 카르비도파, 1 % GAA, LPC, SGDC 및 STDHF 등의 투과 촉진제류의 효과는 거의 없었다. 4. 액상조성물의 설계, 동결건조물의 제조 및 평가 레보도파의 액상조성물 (10 mg/mL)을 설계하기 위하여 용해도가 12.1 mg/mL 이었던 0.1 mol/L 말레산용액을 용제로 선택하였으며 레보도파의 pH 의존적인 용해성을 고려하여 트리에탄올아민을 써서 pH를 조정하였다. 그 결과 pH 3 부근에서는 8 mg/mL의 레보도파 함유 액상조성물을 만들 수 있었으며 pH 2.8-2.5에서 10 mg/mL의 레보도파 함유 액상조성물을 만들 수 있었다. 점막 부착성을 부여하기 위하여 카르보폴 971 또는 HPCD를 첨가하여 본 결과 카르보폴 971을 사용한 조성물은 모두 유백색의 반투명 용액이었으며 카르보폴 971 대신 HPCD를 첨가한 조성물은 미황색의 투명한 용액이었다. 0.1 mol/L 말레산-트리에탄올아민용액 (pH 2.7) 20 mL에 레보도파 200 mg 및 카르비도파 50 mg을 넣고 카르보폴 250 mg 또는 HPCD 2 g을 첨가하여 제조한 액상 조성물의 비강 점막 투과 flux는 각각 494 ± 15 및 503 ± 46 μg/㎠/hr이었으며 lag time은 각각 -0.1 과 2.4 분으로 매우 신속한 투과 특성을 보여 주었다. 카르보폴을 첨가한 액상조성물을 동결 건조하여 얻은 건조물은 점착성이고 딱딱하였으며, HPCD를 함유하는 액상조성물은 동결 건조한 것은 유동성의 미세한 가루로 얻어졌다. HPCD 함유 동결건조물은 당초의 액상조성물과 유사한 함량 및 pH를 나타내었으며 30 ℃에서 20 일까지 레보도파 단독과 동일한 보존 안정성을 나타내었다. 또 HPCD 함유 액상조성물 및 이를 동결 건조하여 물을 넣어 같은 용량으로 녹인 용액 100 μL (레보도파 1 mg 및 카르비도파 0.25 mg 함유)씩을 가지고 비강 점막 투과 시험을 하여 얻은 flux는 각각 503 ± 46 및 470.4 ± 59 μg/㎠/hr이었으며 양자 간에 유의성 있는 차이는 없었다. 이 동결건조물 용액의 pH가 약 2.5인 점을 고려하여 토끼의 비강 점막에 2 시간까지 노출하여 점막손상을 현미경으로 관찰한 결과 비교로 pH 6.8 등장인산염완충액에 동일하게 노출한 것과 같이 수분에 의한 팽윤 현상은 있었으나 점막의 손상이나 박리는 관찰되지 않았다. 5. 레보도파 동결건조물의 비강 투여시의 혈중 및 뇌중 약물동태 동결건조물을 물에 녹여 레보도파로서 10 mg/mL로 하고 레보도파/카르비도파 2.5/0.625 mg/kg의 용량으로 비강에 점적하여 투여하여 혈중 및 뇌중 약물동태학적 파라미터를 구하였다. 비강투여 후 혈중 AUC0→∞ 및 Cmax는 각각 329.7 ± 87.9 ng?hr/mL 및 626.9 ± 147.0 ng/mL)이었다. 비교로 레보도파/카르비도파 80/20 mg/kg의 용량으로 1 회 경구 투여한 후의 혈중의 AUC0→∞ 및 Cmax는 각각 3265.5 ± 542.2 ng?hr/mL 및 520.9 ± 167.8 ng/mL이었다. 한편 비강투여 후 뇌중 AUC0→∞ 및 Cmax는 각각 177.4 ± 87.0 ng?hr/mL 및 388.2 ± 48.4 ng/mL)이었으며 경구 투여 후 뇌중 AUC0→∞ 및 Cmax는 각각 1363.6 ± 361.1 ng?hr/mL 및 268.6 ± 77.0 ng/mL이었다. 동일한 투여 용량으로 환산하여 약물동태학적 파라미터를 비교할 때 동결건조물의 재용해액의 비강투여는 경구투여에 비하여 뇌중 레보도파의 AUC0→∞는 4.2 배, 뇌중 레보도파의 Cmax는 46.2 배 높은 것으로 산출되었다. 이상의 주요 실험 결과는 아침 기상, 외출 및 여행 도중 갑작스러운 레보도파의 혈중 농도 저하로 인한 파킨슨 증상 발현 시 신속하고 편리하게 사용할 수 있는 응급용 제형으로서 레보도파의 비강 점막 투여용 제형 개발을 위한 기초적인 연구 자료가 된다고 사료된다.

The aim of this study was to design and evaluate intranasal formulation of levodopa (LD) and carbidopa. LD can be transported into the brain through the blood-brain barrier where it undergoes decarboxylation to dopamine within the brain and exerts its effect. The oral bioavailability of LD alone is estimated to be about 5 ~ 15 %, and less than 1 % of the administered oral dose reaches the brain unchanged. In this study, various attempts were carried out to enhance the solubility, investigate enzymatic stability of LD in nasal, duodenal and rectal mucosal sites, and enhance the permeation of LD through nasal mucosa. Based on solubility, stability and permeation studies, liquid and its freeze-dried powders were prepared and evaluated in terms of stability, mucosal permeation and damage, and pharmacokinetic behaviors. In solubility studies, various acids were used as additives. In the physicochemical stability study, pH-rate profile was obtained. In the enzymatic stability, mucosal extracts were prepared by leaching the excised nasal, duodenal and rectal mucosa in isotonic phosphate buffers at 37 ℃. The degradation of LD in various mucosal extracts in the absence or presence of carbidopa, decarboxylase inhibitor, was determined. In permeation studies, freshly-excised nasal mucosa was mounted over the cell-opening (0.64 cm²) of Franz-type permeation system. Isotonic phosphate buffer (IPB, pH 6.8, 5 mL) was added into the receptor cell, and 100 μL of donor solutions (0.5 ~ 4 mg/mL of LD and 0.125 ~ 1 mg/mL of carbidopa in pH 6.0 IPB) was put onto the surface of nasal mucosa in the donor cell. 1 % glycyrrhizic acid ammonium salts (GAA), L-α-lysophosphatidylcholine (LPC) and sodium taurodihydrofusidate (STDHF) were used as enhancers. The receptor solution was magnetically stirred at 600 rpm at 37 ℃, and aliquots of 100 μL were removed from receptor cell. The samples were then analyzed for LD and/or CD using the HPLC method. Various liquid formulations were prepared by dissolving LD (1.0 %) and/or carbidopa (0.25 %) in 0.1 mol/L maleic acid solutions and adding carbomer and/or 2-hydroxy-β-cyclodextrin (HPCD), and the pH of solutions was adjusted to pH 2.5 - 2.7 with triethanolamine. Powder formulations containing LD and CD were prepared by freeze-drying liquid formulations. With these dry formulations, the stability was assessed by determining the residual contents at 30 and 45 ℃. The transnasal permeation was studied using Franz-type diffusion system. In vivo study, 250 μL/kg (2.5 mg/kg of LD) of reconstituted solution of freeze-dried powder was administered into both nostrils of rats using a microsyringe. For the contrasts, 48 rats (SD, 250 g, male) were given a single dose of LD/CD (80/20 mg/kg). For the brain and plasma sample, the animal was sacrificed at the desired time and the brain was carefully removed and weighed. Blood samples were directly collected from heart. Brain and plasma concentrations of LD after nasal administrations were compared with those by oral route. From above studies, following results were obtained. Various organic acids improved the solubility of LD concentration-dependently. The addition of EDTA (0.01 %) or sodiumbisulfite (0.1 %) was not significant in stabilizing LD in 0.04 mol/L phosphoric acid and 2 mol/L maleic acid?triethanolamine solutions (pH 4.0). In the nasal, duodenal and rectal extracts, LD was degraded concentration-dependently and percents (%) remaining of LD were 86.3 ± 1.7, 81.8 ± 5.6, and 88.1 ± 0.8 %, respectively, after 5 min-incubation at 20 μg/mL of LD. At higher dose of 100 μg/mL, the degradation of LD was not significant even after 1 hour incubation. In the mucosal extracts, the addition of carbidopa (5 μg/ml) did not significantly affect the enzymatic degradation of LD (20 μg/ml). In permeation studies, the fluxes of LD through nasal, duodenal and rectal mucosa were found to be 145.4 ± 17.4, 19.8 ± 4.7 and 3.9 ± 0.2 μg/cm²/hr, respectively. These results suggest that the enzymatic degradation of LD at the site of mucosa may play an important role on reduced and irregular absorption in accordance with drug concentration, and thus the nasal route would be promising in the fast delivery of LD. From stability studies, it was found the liquid formulations were somewhat unstable, but freeze-dried ones were very stable at 30 ℃, which were comparable to LD alone. In vivo study with rat, the nasal administration of reconstituted solution (LD 2.5 mg/kg) resulted in an improved bioavailability compared to that obtained from oral route of LD and carbidopa (80/20 mg/kg). AUC0→∞ and Cmax in brain after nasal administration (2.5 mg/kg of LD) were 177.4 ± 87.0 ng?hr/mL and 388.2 ± 48.4 ng/mL. AUC0→∞ and Cmax in brain from oral route (80 mg/kg of LD) were 1363.6 ± 361.1 ng?hr/mL and 268.6 ± 77.0 ng/mL. Tmax in the brain after nasal and oral administraion were 13.8 and 57 min, respectively. The AUC0→∞ of LD in the brain after nasal administration was 4.2-fold higher than that from oral route. The Cmax of LD in the brain after nasal administration was 46.2-fold higher than that from oral route. From overall results, it was suggested that the re-constitutable freeze-dried LD-cabidipa powder might provide a stable and ready-use nasal formulation and a means for the fast onset of action in Parkinson's patients. The aim of this study was to design and evaluate intranasal formulation of levodopa (LD) and carbidopa. LD can be transported into the brain through the blood-brain barrier where it undergoes decarboxylation to dopamine within the brain and exerts its effect. The oral bioavailability of LD alone is estimated to be about 5 ~ 15 %, and less than 1 % of the administered oral dose reaches the brain unchanged. In this study, various attempts were carried out to enhance the solubility, investigate enzymatic stability of LD in nasal, duodenal and rectal mucosal sites, and enhance the permeation of LD through nasal mucosa. Based on solubility, stability and permeation studies, liquid and its freeze-dried powders were prepared and evaluated in terms of stability, mucosal permeation and damage, and pharmacokinetic behaviors. In solubility studies, various acids were used as additives. In the physicochemical stability study, pH-rate profile was obtained. In the enzymatic stability, mucosal extracts were prepared by leaching the excised nasal, duodenal and rectal mucosa in isotonic phosphate buffers at 37 ℃. The degradation of LD in various mucosal extracts in the absence or presence of carbidopa, decarboxylase inhibitor, was determined. In permeation studies, freshly-excised nasal mucosa was mounted over the cell-opening (0.64 cm²) of Franz-type permeation system. Isotonic phosphate buffer (IPB, pH 6.8, 5 mL) was added into the receptor cell, and 100 μL of donor solutions (0.5 ~ 4 mg/mL of LD and 0.125 ~ 1 mg/mL of carbidopa in pH 6.0 IPB) was put onto the surface of nasal mucosa in the donor cell. 1 % glycyrrhizic acid ammonium salts (GAA), L-α-lysophosphatidylcholine (LPC) and sodium taurodihydrofusidate (STDHF) were used as enhancers. The receptor solution was magnetically stirred at 600 rpm at 37 ℃, and aliquots of 100 μL were removed from receptor cell. The samples were then analyzed for LD and/or CD using the HPLC method. Various liquid formulations were prepared by dissolving LD (1.0 %) and/or carbidopa (0.25 %) in 0.1 mol/L maleic acid solutions and adding carbomer and/or 2-hydroxy-β-cyclodextrin (HPCD), and the pH of solutions was adjusted to pH 2.5 - 2.7 with triethanolamine. Powder formulations containing LD and CD were prepared by freeze-drying liquid formulations. With these dry formulations, the stability was assessed by determining the residual contents at 30 and 45 ℃. The transnasal permeation was studied using Franz-type diffusion system. In vivo study, 250 μL/kg (2.5 mg/kg of LD) of reconstituted solution of freeze-dried powder was administered into both nostrils of rats using a microsyringe. For the contrasts, 48 rats (SD, 250 g, male) were given a single dose of LD/CD (80/20 mg/kg). For the brain and plasma sample, the animal was sacrificed at the desired time and the brain was carefully removed and weighed. Blood samples were directly collected from heart. Brain and plasma concentrations of LD after nasal administrations were compared with those by oral route. From above studies, following results were obtained. Various organic acids improved the solubility of LD concentration-dependently. The addition of EDTA (0.01 %) or sodiumbisulfite (0.1 %) was not significant in stabilizing LD in 0.04 mol/L phosphoric acid and 2 mol/L maleic acid?triethanolamine solutions (pH 4.0). In the nasal, duodenal and rectal extracts, LD was degraded concentration-dependently and percents (%) remaining of LD were 86.3 ± 1.7, 81.8 ± 5.6, and 88.1 ± 0.8 %, respectively, after 5 min-incubation at 20 μg/mL of LD. At higher dose of 100 μg/mL, the degradation of LD was not significant even after 1 hour incubation.
