PURPOSE: The purpose of this study was to investigate the risk of diabetes mellitus(DM) incidence with relative handgrip strength(HGS) among Korean adults. It is intended to provide meaningful data that can contribute to reducing the prevalence of DM by considering HGS in intervention programs designed to prevent and manage DM. METHODS: A total of 5,657 subjects(2,554 men and 3,103 women) aged ≥20 years were analyzed using the 2019 data of the National Health and Nutrition Examination Survey(KNHANES). Relative HGS was defined as the absolute HGS divided by height squared with the highest correlation coefficient between body measurements. Relative HGS was divided into quartiles, and this study was analyzed stratified by sex, age, and obesity. To investigate the association of DM with HGS, logistic regression analys were done after adjusting for confounders. RESULTS: The prevalence of DM was 14.7%. After adjusting for confounders, the risk of DM increased according to the decrease in quartiles in the less than top 50% of relative HGS(Ptrend=0.002). In obese men and women over 50 years of age, compared with the 4th quartile group with the highest relative HGS, 1st quartile group with the weakest relative HGS was higher risk of DM incidence(men: 1.97 times 95% CI 1.07-3.64, women: 2.15 times 95% CI 1.26-3.64). CONCLUSIONS: The findings indicated that relative HGS was independently associated with the risk of DM incidence in Korean adults. HGS can be used as a predictor of DM because relative HGS measurement may be useful to identify individuals at increased risk for DM incidence. Maintaining a high level of relative HGS is important strategies for DM prevention among adults.
