. 1969년 중국 山西運城의 焦順發이 대뇌피질의 구회를 근거로 하여 두피에 刺鍼한 결과 일련의 뇌혈관계통의 질병에 효과가 있다는 것을 찾아냄으로써 연구 개발된 것이다. 이는 50년대에 시작되어 70년대 초 陜西, 山西, 上海, 南京 등지의 침구연구가들이 두피부위에서 단순히 발현되는 경혈과 대뇌피질층 기능이 두피외표에 투영되는 상응구를 연계시켜 두피의 특정혈위를 鍼刺함으로써 전신성 및 뇌원성 질병을 치료하게 되었다 20).
치매의 頭鍼치료에 대한 임상 논문 선별과정에서, 頭鍼치료와 電鍼치료를 병행한 연구, 頭鍼치료와 양약치료 및 한약치료 등 기타 치료를 병행한 연구도 포함하였다. 검색된 논문에서 임상논문으로 증례연구와 임상시험을 포함하고자 하였으며, 시험관내 및 동물 실험연구, 종설 및 개인 학설에 대한 논문은 제외하고자 하였다.
그러나 아직까지 확실한 기전에 대한 설명이 이루어지지 않았으며, 논란이 많은 가운데, 여러가지 독특한 혈구선정과 刺鍼수법이 존재하고 있어, 현재까지 우리나라에서는 頭鍼療法에 관한 연구가 매우 소수이며, 특히 치매에 대한 연구는 부족한 상태이다. 더욱 임상적 활용방안에 대한 연구는 없었던 관계로, 본 연구에서는 CAJ 상 치매의 頭鍼치료 임상연구 논문에 대한 고찰을 통해 현재 중국에서의 頭鍼療法 임상활용을 살펴보고, 이를 토대로 활용방안을 제시하고자 하였다.
제안 방법
“痴呆” 幷且 “头针”으로 검색하여 나온 논문을 1단계 제목 및 초록 검토를 통해 먼저 논문을 선별하였으며, 2단계 전문 검토를 통해 치매의 頭鍼치료 임상연구에 적합한 논문을 선별하였다.
MRI를 활용한 경우는 1편45)으로 MRI상에서 측렬(lateral fissure) 폭, 제3뇌실 폭, 평균 뇌구(sulcus)폭, 액각지수(관자놀이 길이와 동일 수평상의 머리뼈 길이의 비율), 미상핵지수(관자놀이 길이와 측뇌실3각의 맥락총 석회화간격의 비), 측뇌실체지수(양측실요부 외측벽간 최소거리와 수평내판간 거리를 측정, 비교하였다.
각 연구에서 활용한 頭鍼치료의 방법을 ①혈위 ②침규격 ③刺鍼방법 ④電鍼방법 ⑤염전방법 ⑥ 치료기간 ⑦병행치료로 요약하였다.(Table 2.
대조군은 대부분이 서양약물치료한 경우로 12편32,35,36,37,38,39,40,43,44,45,47,48) 나머지는 正經鍼 46), 正經鍼에 電鍼을 배합한 경우34), 중약치료41)를 대조군으로 하였고, 두 경우는 치료군과 치료방법은 같으나 유침시간을 3, 4시간인 치료군에 비해 30분으로 짧게 하였다. 서양약물치료한 경우를 살펴보면 nimodipine이 가장 많이 쓰인 약물로, 40mg은 2편35,39), 20mg은 4편36,37,43,44), 30mg은 1편45)이었으며, Duxil은 3편32,38,40)으로 그 다음이 었고, 그 외에 약물은 Nicergoline48)과 Ischolium retard47) 로 각각 1편씩이었다.
먼저 증례보고에서는 頭鍼에 項鍼, 正經을 활용한 體鍼, 양방약물치료(diphenylmethylpiperazine 유도체, Duxil), 한약치료(黃芪桂枝五物湯加減方, 通心絡交囊, 凉血淸腦顆粒)을 병행하였다. RCT 연구에서 치료군이 2개인 경우 7편32,36,37,43,44,47,48), 그 외 치료군이 1개인 경우 11편31,33,34,35,38,39,40,41,42,45,46)이었다.

대상 데이터

3. 유효성 평가는 MMSE, HDS, BDS, CCSE, ADL, FAQ, NIHSS, NFD, Webster Rating Scale Score등의 인지기능, 일상생활능력, 신경기능 관련 평가도구가 쓰였으며, 그 외 MRI, P300, 혈장 SS, AVP, TFT(serum T3, T4, free T3)의 영상, 혈액검사를 사용하는 연구도 있었다.
4. 치료결과는 치료군, 대조군 모두 치료 전에 비해 치료 후가 개선되었으며, 대부분 頭鍼 치료를 시행한 군에서 인지기능 및 일상생활 능력 평가 상 대조군보다 유의하게 개선 되었다.
5. 頭鍼치료방법은 焦氏頭鍼法을 배합한 경우와 正經鍼과 經外寄穴을 배합한 경우, 標準頭鍼法을 배합한 경우 세 가지로 분류할 수 있다. 이 중 많이 사용된 혈은 運動區, 感覺 區, 暈聽區, 足運感區, 四神聰, 百會, 神庭, 風池, 額中線(MS1), 頂中線(MS5), 頂顳前斜線(MS6), 頂顳後斜線(MS7)이었다.

후속연구
따라서 본 연구에서는 頭鍼 치료에 대해 더욱 다양한 연구가 보고되고 있는 중국의 임상연구 논문에 대한 고찰함으로써, 향후 치매 치료의 다양화에 기여할 수 있을 뿐 아니라, 특히 頭鍼치료에 있어서의 기초자료로 활용될 수 있을 것으로 기대된다.
이를 기본으로 혈관치매 또는 파킨슨병 등의 경우에서 상하지 마비나 감각이상 등의 신경계 이상이 동반될 때 感覺區, 足運感區, 運動區의 上 3/5, 頂顳前斜線 上 3/5, 頂顳後斜線등을 함께 병행할 수 있으리라 생각한다. 본 연구는 향후 치매에 있어서 한의학적 치료의 다각화와 頭鍼치료의 임상적 활용에 참고기반이 될 수 있으리라 기대한다.
이상에서 살펴본 穴位를 토대로 額中線(MS1)과 頂中線(MS5)은 神庭, 百會, 前神聰부위와 중복되므로 神庭, 百會, 四神聰, 風池를 기본으로 주로 치매환자는 언어기능 저하가 함께 나타나므로 두면 언어운동기능에 자극을 줄 수 있는 運動區의 下 2/5, 頂顳前斜線 下 2/5 양측 혈을 임상에서 활용가능하리라 사료된다. 이를 기본으로 혈관치매 또는 파킨슨병 등의 경우에서 상하지 마비나 감각이상 등의 신경계 이상이 동반될 때 感覺區, 足運感區, 運動區의 上 3/5, 頂顳前斜線 上 3/5, 頂顳後斜線등을 함께 병행할 수 있으리라 생각한다.
이상의 결과를 토대로, 치매환자의 인지기능과 일상생활기능을 개선시키기 위하여 神庭, 百會, 四神聰, 風池를 기본으로 두면 언어운동기능에 자극을 줄 수 있는 運動區의 下 2/5, 頂顳前斜線下 2/5 양측 혈을 배합하는 침치료를 임상에서 활용가능 할 것으로 사료된다.
