제약기업은 의약품의 허가외사용에 대해 어떤 경향을 보이는가? 처방자는 허가 여부를 떠나 임상적 필요에 의한 의약품 처방권한을 주장하여 왔으나, 점점 높아지는 근거중심의학에 대한 요구에 직면하여 허가외사용과 관련하여 과학적 근거를 제시해야 하는 어려움과 마주하게 되었다. 제약기업은 이익창출을 위한 시장 확장에 대한 열망은 있으나, 그 시장이 충분히 크지 않을 경우 허가외사용을 허가 받기 위한 임상시험 수행에 소극적인 경향이 있으며, 대중은 새로운 최신 의약품을 이용하여 치료가 어려웠던 질병을 극복하고 싶은 욕구와 안전하고 과학적 근거가 충분한 의약품을 경제적으로 지불 가능한 수준으로 이용하고 싶다는 기대를 동시에 나타내고 있다.
