Purpose: This study was performed to investigate the effects of moral sensitivity and ethical values on biomedical ethics awareness of nursing students. Methods: A descriptive survey design was used for this study. The participants were 324 nursing students from S University located in J city. The data were collected through a questionnaire survey done from September 30 to October 18, 2014. Data were analyzed using a t-test, ANOVA, Scheffe's test, Pearson correlation coefficients, and stepwise multiple regression. Results: The factors influencing nursing students' biomedical ethics awareness were identified as moral sensitivity(${\beta}=.34$), ethical values(${\beta}=.11$), awareness of Korean nurses' declaration of ethics(${\beta}=.15$), religion(${\beta}=.14$), and religious activity(${\beta}=.12$). Five factors explained 10.8% of nursing students' biomedical ethics awareness. Conclusion: The results of this study can be used to develop further educational programs on the moral sensitivity and ethical values for enhancement of nursing students' biomedical ethics awareness.

자료수집의 정확성을 기하기 위해 자료수집 전에 연구책임자가 설문내용과 자료 수집 방법에 대하여 연구조사원 훈련을 시행하였다. 자료 수집 방법은 연구조사원이 연구대상자에게 연구의 목적, 설문지 작성 요령, 설문지 작성과정에서 발생 가능한 다양한 문제가 발생될 때는 설문작성을 중단할 수 있음을 충분히 설명한 후 자발적인 참여의사를 밝힌 대상자에 한하여 서면동의서를 받은 후 자기기입 방식으로 설문조사를 실시하였다. 설문조사 과정은 평균 15분 정도 소요되었으며, 설문이 끝난 즉시 설문지는 곧바로 회수하였다.

대상 데이터

본 연구의 대상은 C도 J시에 소재한 일개 대학교에 재학 중인 2, 3, 4학년 간호대학생에게 본 연구의 취지와 목적을 설명한 후 설문에 응답할 것을 허락하고 동의서에 서명한 학생에게 설문지를 주고 응답하게 하였다. 간호학과 1학년은 연구동의서에 스스로 동의를 할 수 없는 연령이어서 대상자에서 제외하였다.
설문조사 과정은 평균 15분 정도 소요되었으며, 설문이 끝난 즉시 설문지는 곧바로 회수하였다. 총 340부 중에서 불성실하게 응답한 16부를 제외한 324명의 자료가 분석에 이용되었다.

이론/모형

도덕적 민감성 측정도구는 Lutzen, Evertzon과 Nordin (1997)이 개발하고 Han, Kim, Kim과 Ahn (2010)이 한국판으로 수정 보완한 도구를 사용하였다. 본 도구는 총 27문항으로 환자 중심간호 5문항, 전문적 책임 7문항, 갈등 5문항, 도덕적 의미 6문항, 선행영역 4문항으로서 ‘매우 그렇다' 5점, ‘그렇다' 4점, ‘보통이다' 3점, '그렇지 않다' 2점, ‘전혀 그렇지 않다' 1점의 척도로서 점수가 높을수록 도덕적 민감성이 높음을 의미한다.
생명의료윤리 인식측정도구는 Lee (2002)의 29문항과 Choi (2002)의 25문항 설문지를 Kwon (2003)이 재구성한 도구를 사용하였다. 도구는 총 49문항으로서 태아생명권 5문항, 인공임신중절 6문항, 인공수정 7문항, 태아진단 5문항, 신생아생명권 5문항, 안락사 5문항, 장기이식 4문항, 뇌사 5문항, 인간생명공학 7문항으로 구성되어 있으며, 각 문항은 4점 척도로 ‘찬성’ 4점, ‘대체로 찬성’ 3점, ‘대체로 반대’ 2점, ‘반대’ 1점으로 부정문항은 역산 처리하였으며, 점수가 높을수록 인간생명을 존중하는 생명의료윤리 인식이 높음을 의미한다.

성능/효과

그리고 일반적 특성에 따른 윤리적 가치관과의 차이를 분석한 결과 학년, 전공만족도, 간호사 윤리강령 인식정도, 임상실습 경험유무에 따라 차이가 있었다. 3학년보다는 2학년에서, 전공에 만족하는 경우, 간호사 윤리강령을 알고 있는 경우에서 윤리적 가치관이 높았다.
본 연구에서 간호대학생의 윤리적 가치관을 공리주의와 의무주의로 구분하여 그 경향을 알아본 결과 대상자들은 윤리적 가치관의 전 영역에서 모두 의무주의 경향을 보였다. 간호 학생들은 간호사로서의 지켜야할 윤리적 책무성을 중요하게 인식하고 있으며, 윤리적 가치관은 어떤 상황에서라도 반드시 지켜야한다는 의무주의적 사고를 하고 있음을 의미하는 것이다.
생명의료윤리 인식에 영향을 미친 요인은 도덕적 민감성, 윤리적 가치관, 종교유무, 종교 활동, 간호사 윤리강령 인식정도이었으며(F=14.19, p<.001), 이들 변수의 설명력은 10.8%이었다(Table 4).

후속연구

본 연구에서 간호대학생의 생명의료윤리 인식에 영향을 미친 요인들은 도덕적 민감성, 윤리적 가치관, 간호사 윤리강령 인식정도, 종교유무, 종교 활동이었으며, 연구대상자의 윤리적 관점은 의무주의 경향을 보여주었다. 따라서 간호대학생의 생명의료윤리 인식을 확고하게 확립하기 위해서는 생명의료윤리교과목의 개설 및 운영, 윤리적 사례분석을 통한 윤리문제의 인식 및 해결방안 모색을 통한 윤리적 가치관의 확립, 그리고 도덕적 민감성 증진을 위한 ‘공감적 듣기’, ‘역할극’, 문학작품이나 영화 등을 활용한 도덕적 상상력을 키워주는 전략이 필요하다고 하겠다.
