목적: 본 연구의 목적은 간호대학생의 임종간호 태도에 영향을 미치는 요인을 조사하기 위함이다. 방법: 대상자는 B광역시에 소재하는 일 대학교에 재학 중인 간호대학생 175명이었으며, 자가보고식 설문지를 이용하여 자료를 수집하였다. 자료는 SPSS/WIN 22.0을 이용하여 t 검정, ANOVA 및 피어슨 상관관계분석과 단계적 다중회귀분석으로 분석하였다. 결과: 간호대학생과의 임종간호 태도는 의사소통능력(r=0.438, P<0.001), 영성(r=0.374, P<0.001), 감사성향(r=0.318, P<0.001)이 유의한 상관관계가 있었다. 임종간호 태도에 영향을 미치는 주요 요인은 의사소통능력으로서 설명력은 18.7%였다. 결론: 이상의 결과로 의사소통능력이 간호대학생의 임종간호 태도에 영향을 미치는 요인임을 알 수 있었다. 추후 간호대학생의 임종간호 태도를 향상시키기 위해 의사소통능력을 강화하는 프로그램을 개발하는 것이 필요하다.

간호사의 영성이 임종간호 태도와 관계가 있다고 한 연구들(3,6,13,15)도 있지만 임종 간호 태도와 관계가 없다고 한 연구(2)도 있으며, Feudtner 등(14)은 감사성향과 같은 긍정적 정서가 임종 간호 수행에 긍정적 영향을 미침을 보고하였으나 Chi와 Kim(2)은 정신건강이 임종간호 태도와 관계가 있으나 영향을 주는 변수는 아니라고 하는 등 연구결과 간에 일관성이 없었다. 이에 본 연구에서는 개인의 내적 자원인 의사소통능력, 영성, 감사성향과 간호대학생의 임종간호 태도의 관계를 파악하여 간호대학생이 긍정적인 임종간호 태도를 확립하는데 도움이 되는 중재를 개발하는데 기초자료를 마련하고자 하였다.
제안 방법
자료 수집은 2014년 9월 1일부터 2015년 6월 12일까지 B광역시에 소재하는 일 간호대학에서 시행하였다. 보조 연구원이 연구 참여자에게 연구의 목적과 필요성,연구 참여자의 권리와 자료 관리에 대하여 설명한 후자발적으로 참여하기로 동의한 대상자에게 설문지를 배포하여 스스로 작성하도록 하였으며 설문 작성에는 약 15분 정도 소요되었다. 설문에 응한 간호대학생에게는 감사의 표시로 소정의 선물을 제공하였다.
대상 데이터
대상자는 B광역시에 소재하는 4년제 간호대학에 재학 중인 3, 4학년 학생 중 연구의 목적을 이해하고 자발적으로 연구참여에 서면 동의한 175명이었다. 대상자 수는 G Power 3.
데이터처리
대상자의 일반적 특성, 의사소통능력, 영성, 감사성향, 임종간호 태도는 빈도와 백분율, 평균과 표준편차로 제시하였다. 대상자의 일반적 특성에 따른 임종 간호 태도의 차이는 t-test와 ANOVA로 분석하였다. 임종간호 태도와 변수들간의 상관관계는 피어슨 상관관계분석으로 파악하였으며, 임종간호 태도에 영향을 미치는 요인은 단계적 다중회귀분석으로 파악하였다.

이론/모형
3) 감사성향: 감사성향은 McCullough 등(20)이 개발하고 Kwon 등(21)이 번안하여 타당화한 한국판 감사 질문지(Gratitude Questionnaire-6, GQ-6)로 측정하였다. 이 도구는 총 6문항의 7점 척도로 점수가 높을수록 감사성향이 높은 것으로 해석한다.
2) 영성: 영성은 Howden(18)이 개발하고 Oh 등(19)이 한국어판으로 번역하여 타당도와 신뢰도를 검증한 도구로 측정하였다. 이 도구는 총 28문항의 5점 척도로 삶의 의미와 목적, 내적 자원, 관계성, 초월성의 4개 하위영역으로 구성되어있다.
1) 의사소통능력: 의사소통능력은 Rubbin(16)이 개발하고 Hur(17)가 국내에 맞게 수정 보완한 포괄적 대인 의사소통능력 척도(Global Interpersonal Communication Competence Scale, GICC)로 측정하였다. 이 도구는 총 15 문항의 5점 척도로 각 문항에 대하여 ‘전혀 그렇지 않다’ 1점부터 ‘매우 그렇다’ 5점까지 응답하도록 되어있으며, 점수가 높을수록 의사소통능력이 높음을 의미한다.
4) 임종간호 태도: 임종간호 태도는 Frommelt(9)가 개발한 Attitudes Toward Care of the Dying Scale (FATCOD)을 Cho와 Kim(10)이 수정ㆍ보완한 도구로 측정하였다. 이 도구는 긍정 15문항과 부정 15문항으로 구성되어 있으며, 각 문항에 대하여 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 4점까지 응답하도록 하여, 점수가 높을수록 임종간호 태도가 긍정적임을 의미한다.
성능/효과
본 연구의 결과 간호대학생의 임종간호 태도에 영향을 미치는 주요 요인은 의사소통능력이었는데, 설명력은 18.7%였다. 간호사는 임종간호 시 환자와 가족들의 요구에 적절히 반응하고 가족의 역동과 환자와 가족이 표현하지 않은 내면의 갈등을 인지하여 효율적으로 대처할 수 있는 의사소통능력이 요구된다(5).
본 연구의 결과 의사소통능력이나 영성, 감사성향이 높을수록 임종간호 태도가 긍정적이었다. 즉, 타인과 공감적 의사소통 경험이 높을수록, 현재의 삶의 의미와 가치에 대해 숙고하고 주변과 조화로운 관계를 이루어갈수록, 삶의 과정에서 긍정적 신념이 높을수록 임종 간호에 대한 이해가 높고 긍정적인 태도를 형성하게 됨을 알 수 있었다.
후속연구
이상의 결과에서 간호대학생의 임종간호 태도는 의사소통능력, 영성, 감사성향과 상관관계가 있음을 알 수 있었으며, 특히 의사소통능력은 임종간호 태도에 영향을 미치는 주요 요인이었으므로 간호대학생들의 의사소통능력을 향상시킬 수 있는 관계 중심 모델의 교육프로그램을 개발하고 적용하는 것이 필요하다. 본 연구결과를 토대로 임종간호 태도에 영향을 미치는 요인으로 확인된 의사소통능력을 향상시킬 수 있는 중재를 개발하는 연구가 필요하다.
