Purpose: The purpose of this study was to investigate perceptions of adequacy and job performance of the nurse assistants' nursing job as evaluated by registered nurses and nurse assistants in geriatric hospitals, and by caregivers from the same hospitals. Methods: Participants included 62 registered nurses, 57 nurse assistants, and 64 patient caregivers who completed a measurement scale on the job of nurse assistants. Data collection was conducted from October to December, 2015. Data were analyzed using descriptive statistics and one-way ANOVA by IBM SPSS/WIN program version 21.0. Results: There were significant differences by items in perception of adequacy of nurse assistants' nursing job of among the three groups. There was also a significant difference in perception of the nurse assistants' job performance among the three groups. Conclusion: For nurse assistants in geriatric hospitals, the Ministry of Health and Welfare needs to develop an appropriate job practice guideline. In addition, there should be periodic courses of retraining and continuing education for nurse assistants.

설문지 작성에 필요한 소요시간은 15~20분 정도였으며, 설문에 응해 준 대상자에게는 소정의 답례품을 제공하였다. 탈락률을 고려하여 220부를 배부하여 216부가 회수되었고, 그 중 응답이 미흡하거나 불성실하여 자료처리가 어려운 33부를 제외한 총 183부가 최종분석에 사용되었다.

• 간호사, 간호조무사, 환자보호자가 인식하는 간호조무사의 간호업무 적절성과 업무수행여부에 대한 차이는 One-way ANOVA로 분석하였으며, 사후 검증은 Scheffé test로 하였다.

이론/모형

간호조무사의 간호업무 수행여부에 대한 인식을 측정하기 위하여 Back과 Kim[14]이 개발한 간호조무사의 업무 도구 40문항을 사용하였고 원 저자에게 도구 사용에 대한 승인을 받았다. 각 문항은 ‘수행한다’ 1점과 ‘수행하지 않는다’ 0점으로 구분하여 응답하게 하였다.
간호조무사의 간호업무 적절성을 알아보기 위하여 Back과 Kim[14]이 개발한 간호조무사의 업무 측정도구를 사용하였다. 본 연구에 앞서 저자에게 도구 사용에 대한 승인을 받았다.

성능/효과

간호업무 수행여부에 대한 인식에서는 간호사와 간호조무사 및 환자보호자의 인식에 유의한 차이가 있었다. 간호사와 간호조무사는 간호조무사가 대부분의 업무에 대해 수행하고 있다고 하였으나, 환자보호자들은 간호조무사가 모든 업무를 수행하고 있지는 않다고 인식하고 있었다.

후속연구

첫째, 요양병원은 노인 환자를 대상으로 광범위한 간호업무를 필요하기 때문에 간호조무사의 간호지식 향상과 업무의 질 관리 강화를 위해 주기적인 보수교육과 병원내의 지속적인 교육이 실시되어져야 한다. 둘째, 간호조무사의 업무 규정이 아직은 법령화되지 않았으므로 향후에 국민건강을 위협하지 않는 실질적인 업무범위가 설정이 되어져야 하고, 간호업무를 위임하고 감독하는 간호사와 보조업무를 수행하는 간호조무사에 대한 제도적 보호장치가 마련되어져야 할 필요가 있다.
본 연구는 일부 지역의 대상자만을 선정하였다는 제한점이 있어 향후에는 좀 더 다양한 지역의 요양병원의 간호조무사와 간호사 및 환자보호자를 대상으로 반복연구가 시행되어야 할 것이다. 그러나 요양병원에 근무하는 간호조무사의 간호업무 실태에 대하여 간호사와 간호조무사를 대상으로 분석한 연구는 있으나 환자보호자의 간호업무에 대한 인식을 조사한 연구는 찾기 힘든 실정에서 본 연구는 간호조무사의 간호업무 적절성과 수행여부에 대한 환자보호자의 인식정도까지 파악하였다는데 의의가 있다.
본 연구의 결과를 토대로 다음과 같이 제언한다. 첫째, 요양병원은 노인 환자를 대상으로 광범위한 간호업무를 필요하기 때문에 간호조무사의 간호지식 향상과 업무의 질 관리 강화를 위해 주기적인 보수교육과 병원내의 지속적인 교육이 실시되어져야 한다. 둘째, 간호조무사의 업무 규정이 아직은 법령화되지 않았으므로 향후에 국민건강을 위협하지 않는 실질적인 업무범위가 설정이 되어져야 하고, 간호업무를 위임하고 감독하는 간호사와 보조업무를 수행하는 간호조무사에 대한 제도적 보호장치가 마련되어져야 할 필요가 있다.

	요양병원에 입원하는 노인 환자의 보호자가 요양시설보다는 요양병원을 선택하게 되는 이유는 무엇인가?	노인인구의 증가로 요양병원도 계속 증가하고 있는데 요양병원에 입원하는 노인 환자들의 경우는 장기적인 치료와 요양이 필요한 상태로서 병원을 선택할 때 환자 본인의 의견보다는 주보호자에 의해 결정되는 경우가 많다[15]. 보호자들은 노인의 질병수가 많고, 부양 관련 스트레스가 많을수록 요양시설보다는 요양병원을 선택하게 되는데, 이는 노인에게 치료가 필요하고 일상생활활동에 도움이 필요한 경우이기 때문이다[15]. 그러나 노인요양병원에 입원한 당사자나 가족은 간호수행에 대해서 청결하지 않은 목욕서비스와 체위변경을 하지 않는 것, 적절한 시기에 기저귀 교환이나 린넨 또는 환의교환이 이루어지지 않아 불결함, 사전동의 없이 억제대를 적용함 등의 서비스 미흡성을 지적하였다[16].
