구강 내 균집락과 구강당이 생후 초기 어떤 적응과정을 보이며, 다양한 임상적 상태와 특성에 따라 어떤 차이가 나는지 분석한 결과는 어떠한가? | 본 연구는 NICU 입원 고위험신생아를 대상으로 구강 내 균집락과 구강당이 생후 초기 어떤 적응과정을 보이며, 다양한 임상적 상태와 특성에 따라 어떤 차이가 나타나는지 탐색하기 위해 수행되었다. 그 결과 가장 다빈도로 관찰된 균은 Streptococcus, MRSA, Staphylococcus, CNS였는데 특히 광선치료와 복위 체위는 구강 균집락 위험을 증가시키는 반면 항생제나 비경구영양공급은 균검출 위험을 감소시키는 것으로 나타났다. 구강당은 만삭아, 저혈당이 아닌 경우, 임신성 당뇨가 없는 산모의 경우, 역류가 없는 경우, 경구삽관이 없고, 경구영양공급을 받는 등의 일반적 상황에서는 혈당보다 낮거나 유사한 값으로 나타났다. 고위험신생아의 구강은 호흡과 영양을 위해 외부와 상호작용하는 기관이므로 감염관리 측면에서 정교한 구강간호를 필요로 한다. 본 연구결과에서 제시된 고위험신생아의 임상 특성에 따른 구강당과 균 집락에 대한 탐색 결과는 추후 표준화된 구강간호의 개발과 적용에 유용하게 사용될 것으로 판단된다. |
