중소병원 간호사의 병상규모별 간호업무환경과 직무배태성이 이직의도에 미치는 영향 Effects of Nurses' Practice Environment and Job Embeddedness on Turnover Intention in Medium-small sized Hospital원문보기
본 연구는 중소병원 간호사의 병원규모별 간호업무환경, 직무배태성이 이직의도에 미치는 영향을 확인하기 위함이다. 본 연구는 2015년 8월 10일부터 8월 24일까지 B광역시 소재 200병상 미만 병원에 근무하는 간호사 131명, 200~400병상 병원에 근무하는 간호사 145명을 대상으로 설문지를 이용한 서술적 조사연구이다. 자료분석은 SPSS/WIN 18.0 프로그램으로 분석하였다. 연구결과는 다음과 같다. 첫째, 병상규모 200~400병상 간호사와 200병상 미만 간호사에서 간호업무환경과 직무배태성은 유의한 차이가 없었으나, 이직의도는 유의한 차이가 있었다. 둘째, 두 집단 모두에서 이직의도는 간호업무환경 및 직무배태성과 유의한 음의 상관관계가 있었다. 셋째, 간호사의 이직의도에 영향을 미치는 요인은 200병상 미만 병원에서 직무배태성, 나이, 간호업무환경의 순이었으며, 200~400병상 병원에서 직무배태성, 월급여, 간호업무환경의 순이었다. 현재 중소병원 간호사의 이직률 증가로 인해 간호사의 인력난이 심각한 수준이다. 이에 본 연구결과는 중소병원 간호사의 간호업무환경을 개선하고 직무배태성을 높여 이직의도를 감소시키기 위한 전략을 마련하는데 기초자료로 이용될 수 있을 것이다.

The results were as follows: (a) Nurses' practice environment and job embeddedness showed no significant differences between the two groups, but there were significant differences of turnover intention between the two groups; (b) turnover intention was negatively correlated with nurses' practice environment and job embeddedness in each group; and (c) in hospital of less than 200beds, the factors of influencing turnover intention were job embeddedness, age and nurses' practice environment. Conversely, in a hospital with 200 to 400 beds, the factors of influencing turnover intention were job embeddedness, pay, and nurses' practice environment. This study suggests that to reduce turnover intention, we have to improve the practice environment of nurses and to enhance job embeddedness for nurses in medium-to-small sized hospitals.

따라서, 본 연구는 병원간호사회에서 실시한 병원간호인력 배치현황 실태조사[3]에 나타난 병상규모별 운영 병상 및 간호사 이직률을 토대로, 중소병원을 200병상 미만과 200∼400병상으로 나누어, 간호사의 간호업무환경과 직무배태성이 이직의도에 미치는 영향을 확인함으로써, 중소병원의 간호인력 관리에서 병상 규모에 따른 간호사 이직의도 감소를 위한 전략 마련에 기초자료를 제공하고자 한다.
제안 방법
자료수집 전, 해당 병원의 간호부를 직접 방문하여 연구의 목적과 절차를 설명하고 승인을 받았다. 이후 연구 대상자의 각 해당 병동을 방문하여 수간호사 및 병동간호사에게 연구목적을 설명하고 연구참여에 대한 동의를 받은 후, 설문참여에 대한 개별 서면동의서가 포함된 설문지를 배부하였다. 작성된 설문지는 밀봉된 봉투에 넣도록 하고, 연구자가 다시 방문하여 이를 회수하였다.
대상 데이터
본 연구에서 중소병원의 병상규모는 병원간호사회에서 제시한 병상수별 이직률에 근거하여[3] 이직률이 높은 200병상 미만 및 200∼400병상 규모별로 선정하였다. 연구대상자는 B시 소재 200병상 미만 4개 병원과 200∼400병상 2개 병원에 근무하는 중소병원 간호사로, 본 연구의 목적을 이해하고 자발적으로 연구 참여에 동의한 자를 임의 표출하여 선정하였으며, 이 중 임상경력이 1년 미만인 간호사는 간호업무환경에 대한 판단이 정확하지 않을 수 있어 연구대상에서 제외하였다. 표본수는 G-power 3.
탈락률을 약 10∼15%로 고려 시, 연구에 필요한 대상자수는 병상규모별 145∼152명임을 근거로, 150명의 자료를 수집하였다. 이 중 응답이 불충분한 설문지를 제외하고 최종 분석대상자는 200병상 미만 병원은 131명, 200∼400병상 병원은 145명이었다.

이론/모형
Lake[15]가 개발한 Practice Environment Scale of the Nursing Work Index (PES-NWI)를 Cho, Choi, Kim, Yoo와 Lee[16]가 우리나라 실정에 맞게 번안한 도구를 사용하였다. 본 도구는 총 29문항이며, 5개 하부영역으로 이루어졌다.
Mitchell 등[7]이 개발한 직무배태성 측정 도구를 Kim, Lee와 Choi[17]가 수정·보안한 도구를 사용하였다. 본 도구는 총 18문항이며, 3개 하부영역으로 이루어졌다.
Mobley 등[18]이 개발한 이직의도 측정 도구를 Kim[19]이 수정·보안한 도구를 사용하였다. 본 도구는 총 6문항으로 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점까지의 Likert 5점 척도로 측정하며, 점수가 높을수록 이직의도의 정도가 높음을 의미한다.
성능/효과
이직의도에 영향을 미치는 요인은 200병상 미만 병원에서 직무배태성, 나이 및 간호업무환경이었으며, 200∼400병상 병원에서 직무배태성, 월급여, 간호업무환경이었다. 이 중에서 직무배태성은 두 집단 모두에서 가장 큰 영향요인이었다.
이직의도와 간호업무환경, 직무배태성의 상관관계에서, 두 집단 모두에서 이직의도는 간호업무환경 및 직무 배태성과 유의한 음의 상관관계로, 간호업무경이 좋을수록, 직무배태성이 높을수록 이직의도가 낮았다. 이는 이직의도가 직무배태성과 음의 상관관계를 보인 Jeon과 Yom[8]의 연구 및 이직의도가 간호근무환경과 음의 상관 관계를 보인 Kang[14]의 연구결과와 유사하여, 직무배태성이 높을수록, 근무환경이 좋을수록 이직하려는 생각이 감소하는 것을 알 수 있다.
후속연구
연구결과, 중소병원 간호사의 이직의도에 영향을 미치는 요인으로 직무배태성이 가장 큰 영향요인이었으며, 이외에 간호업무환경, 월급여 및 나이 등이 있었다. 따라서 중소병원 규모별 특성을 고려하여, 간호사의 이직을 감소시키기 위해서 직무배태성을 높이고, 간호업무환경을 개선하기 위한 노력과 더불어 각 연령층의 특성을 이해하고 배려하기 위한 관심이 필요하다. 또한 월급여에 대한 방안도 마련되어야 할 것이다.
앞으로 노인인구 및 만성질환자의 지속적인 증가로 중소병원을 이용하는 환자는 점차 증가할 것으로 예상된다. 이에 반해, 현재 중소병원 간호사 이직률은 매우 높아 인력난은 심각한 수준으로 대안 마련이 요구되고 있어, 추후 중소병원 간호사의 이직의 영향요인을 확인하기 위한 반복연구가 필요할 것으로 사료된다.
