Purpose: The purpose of this study was to develop a tool to evaluate the nursing competency of ambulatory nurses and to verify the reliability and validity of the developed tool for use in general hospitals. Methods: The evaluation tool for ambulatory nursing competency was developed through a literature review and tests for validity and reliability. Results: Subsequent to a review of the literature on nursing performance of ambulatory nurses, a 12 item questionnaire was developed. Through factor analysis, 12 items in a 2 factor solution were derived. Cronbach's ${\alpha}$ coefficient of the final instrument was .86. Conclusion: The assessment tool developed in this study allows for objective assessments of nursing competencies in relation to the competencies expected of ambulatory care nurses. It is hoped that the assessment tool can be used for empirical verification and to provide basic data for establishing the necessary policies to secure outstanding human resources.

대상 데이터

자료수집기간은 2015년 11월 16일부터 11월 30일까지 15일간 G도 소재 1개 종합병원의 외래간호사와 서울 소재 1개 종합병원, K도 소재 1개 종합병원의 외래간호사를 대상으로 하였다. 자료수집을 위한 설문지를 각 병원 간호부에 보내어 자료수집 승인을 받은 후 외래간호팀장에게 연구의 목적을 설명하고 연구진행에 대한 허락을 구하였다.

데이터처리

대상자의 일반적 특성은 빈도, 평균, 백분율로 분석하였으며 간호역량 평가도구의 내용타당도와 구성타당도를 확인하였다. 개발된 도구의 내적일관성 신뢰도 Cronbach's ⍺는coefficients로 확인하였으며, 도구의 타당도는 Varimax 회전과 주요인 분석(principle component analysis)에 의한 탐색적요인분석(exploratory factor analysis)을 실시하였다. 개발된 도구의 준거타당도를 위한 셀프리더십, 간호업무성과 도구와의 관계는 Pearson correlation 분석으로 산출하였다.

이론/모형

본 연구는 외래간호사의 간호역량 평가도구를 개발하고 이의 타당도를 검증함으로써 외래간호사의 간호역량 향상을 위한 평가 도구를 개발하기 위한 방법론적 연구이다. 간호사의 역량에 대한 연구인 본 연구의 개념적 기틀은 Caper [17]가 제시한 간호지식의 4가지 유형 모델을 기반으로 한 Jang [18]의 간호역량 모형을 근거로 하여 외래간호사에게 필요한 역량을 과학적 간호역량군, 윤리적 간호역량군, 인격적 간호역량군, 심미적 간호역량군으로 구분하여 각 역량군별 핵심 역량을 묻는 문항을 구하였다.

성능/효과

본 연구는 외래간호사의 간호역량 증진을 위해 외래 간호사의 간호역량을 평가할 수 있는 도구를 개발하고 개발된 도구의 신뢰도와 타당도를 검증하기 위해 시행되었다. 문헌고찰과 전문가 대상 내용타당도 및 구성타당도 검증 결과 12개 역량으로 확인된 외래간호사 핵심역량은 간호절차, 환자선별, 임상수행능력, 간호의 지속적 관리, 위임, 협력, 환경관리, 지식추구, 질향상, 근거기반 실무, 전문직업성, 의사소통이었으며 이를 바탕으로 정의를 기술하고 문항화한 도구의 신뢰도 Cronbach’s ⍺는 .86으로 확인되었다. 외래간호사의 역량 평가 도구를 위한 개발 이외에 개발된 도구를 통해 확인한 중요도-수행도 측정 결과 지식추구 역량, 질 향상, 위임에 대해서는 교육과 지도가 필요하며 이를 통해 외래간호사의 역량 증진을 위한 방안으로 활용할 수 있을 것이다.

후속연구

외래간호사의 역량 평가 도구를 위한 개발 이외에 개발된 도구를 통해 확인한 중요도-수행도 측정 결과 지식추구 역량, 질 향상, 위임에 대해서는 교육과 지도가 필요하며 이를 통해 외래간호사의 역량 증진을 위한 방안으로 활용할 수 있을 것이다. 따라서 본 연구는 외래 간호사에게 필요로 되는 역량과 해당 역량의 평가 도구를 개발하여 외래 간호사의 역량 개발을 위한 전략 개발 및 인력관리에 도움을 줄 수 있을 것으로 기대하는 바이다.

	외래간호의 특징은?	이러한 외래의 효율적인 간호인력 운영의 필요성은 병원조직이 해결해야 할 과제이며 역량 있는 간호사의 배치를 통한 효과적인 운영은 아무리 강조해도 지나치지 않다[1]. 외래간호는 다른 간호 분야에 비해 간호수행 측면에서도 차별화되는데 특히 짧은 시간동안 예측하기 어려운 수많은 고객의 요구에 대해 즉각적으로 반응해야하며 그러한 요구 또한 신속하게 해결해야 하는 특성을 가지고 있다[4]. 특히 사회적, 법적, 기술 및 보건 산업 동향은 외래 진료 환경의 진료 시스템과 전문간호수행의 변화를 가져와 오늘날 외래 진료는 더욱 정교하고 복잡한 조직 및 시스템으로 발전하였다[1].
