Purpose: This study was conducted to identify the effects of a simulation practicum using standardized patients on nursing student's communication skills, critical thinking disposition and clinical competency. Methods: A one group pretest-posttest design was used. The participants consisted of 45 fourth year nursing students at G university. This study developed 2 scenarios for patients with diabetes mellitus and the simulation was conducted for an average of approximately 14 hours. Data were analyzed with descriptive analysis, t-tests and Cronbach's ${\alpha}$ using SPSS 21.0 program. Results: Communication skills, clinical competency and self-confidence for diabetic care were statistically improved; however, critical thinking disposition was not improved compared to that of pretest. Conclusion: Simulation practicum using standardized patients may be effective in enhancing communication skills, clinical competency and self-confidence for diabetic care among nursing students. In addition, standardized patients simulation program to promote the critical thinking needs to be developed and further research related to standardized patients simulation is in need.

제안 방법

본 연구는 2015년 6∼8월까지 SP 시뮬레이션 실습교육을 개발한 후 2차에 걸쳐 각각 시뮬레이션 실습교육을 운영 및 평가하였다. 1차는 2015년 9월 22일부터 10월 16일, 2차는 2016년 9월 28일부터 10월 19일까지 동일한 절차로 시뮬레이션 실습교육을 운영하고 그 효과를 평가하였다.

대상 데이터

본 연구 대상자는 J시 소재 일 대학교 간호학과의 시뮬레이션 실습 프로그램에 참여한 4학년 전체 학생으로 학생 수가 적어 2년에 걸쳐 연구를 진행하였으며 1차 연도에 22명, 2차 연도에 23명, 총 45명이었다. 대상자는 임상실습 학점을 모두 이수한 4학년 학생으로, 3학년 때에 내분비계 성인 간호학 강의를 동일한 교수로부터 동일한 강의시간 동안 수강하였고, SP 시뮬레이션 교육에 참여한 경험이 없는 학생을 대상으로 하였다.

데이터처리

1) 대상자의 특성은 빈도, 백분율, 평균, 표준편차를 구하였고, 1차 연도 집단과 2차 연도 집단의 동질성 검정은 χ² test 또는 Fisher’s exacts와 independent t-test로 분석하였다.
2) SP 시뮬레이션 실습교육 전후의 의사소통능력, 비판적 사고 성향, 임상수행능력, 당뇨병 간호수행 자신감의 차이는 paired t-test로 분석하였다.

이론/모형

간호대학생의 비판적 사고성향을 측정하기 위하여 Yoon [21] 이 개발한 비판적 사고성향 도구를 사용하였다. 이 도구는 총 27문항 5점 척도로 부정적 문항인 2개 문항은 역으로 환산하여 사용하였고, 7개의 하위영역인 지적 열정/호기심 5문항, 신중성 4문항, 자신감 4문항, 체계성 3문항, 지적 공정성 4문항, 건전한 회의성 4문항, 객관성 3문항으로 구성되었으며, 점수가 높을수록 비판적 사고성향이 높음을 의미한다.
간호대학생의 의사소통능력을 측정하기 위해 Yoo [12]가 전문가 타당도의 검증을 거친 교육프로토콜을 근거로 개발한 의사소통능력 평가도구를 사용하였다. 이 도구는 간호사로서 전문적 태도 2개 문항과 간호 수행 중 충분한 설명을 평가하는 3개 문항으로 구성되었으며 각 문항은 ‘매우 잘한다’ 5점에서 ‘매우 못한다’ 1점의 5점 척도로, 점수가 높을수록 의사소통능력이 높음을 의미한다.
간호대학생의 임상수행능력을 측정하기 위해 Yang과 Park [22] 이 개발한 임상수행능력 측정 도구를 사용하였다. 이 도구는 6개의 하위영역인 간호과정, 직접적 간호중재, 심리⋅사회적 간호, 환자교육, 신체사정과 모니터, 기본간호 수행으로 구성된 총 19문항의 5점 척도 도구이며, 점수가 높을수록 임상수행능력이 높음을 의미한다.

성능/효과

본 연구는 SP를 활용한 당뇨병 간호 시뮬레이션 실습교육이 간호대학생의 의사소통능력, 비판적 사고 및 임상수행능력에 미치는 효과를 검증하기 위한 단일군 사전 사후설계 실험 연구이다. 본 연구결과 SP 시뮬레이션 실습교육은 간호대학생의 의사소통능력과 임상수행능력을 향상시켰으나 비판적 사고성향에는 유의한 차이가 없었다.

후속연구

결론적으로 SP를 활용한 시뮬레이션 실습교육은 간호대학생의 의사소통능력과 임상수행능력을 향상시키는데 효과적인 교육방법임을 확인하였다. 또한 비판적 사고성향을 향상시키기 위해서는 다양한 상황의 시나리오를 적용한 통합 시뮬레이션 프로그램을 개발하여 지속해서 시행하고 장기적으로 그 효과를 조사할 필요가 있다. 본 연구결과를 바탕으로 향후 다양한 시나리오 모듈을 개발하여 SP 시뮬레이션 실습교육에 적용하고 그 효과를 검증하는 연구를 시행할 것을 제언한다.
또한 비판적 사고성향을 향상시키기 위해서는 다양한 상황의 시나리오를 적용한 통합 시뮬레이션 프로그램을 개발하여 지속해서 시행하고 장기적으로 그 효과를 조사할 필요가 있다. 본 연구결과를 바탕으로 향후 다양한 시나리오 모듈을 개발하여 SP 시뮬레이션 실습교육에 적용하고 그 효과를 검증하는 연구를 시행할 것을 제언한다.
