최근 의료기관평가 인증에서 환자안전사고에 대한 예방이 강화 되면서 환자안전은 환자를 중심으로 발생하는 의료사고를 관리하여 환자의 안전을 유지하는 것이다. 따라서 본 연구는 모의상황을 활용한 환자안전 교육프로그램을 개발하고, 입원환자에게 적용하여 교육 전, 후에 따른 환자안전에 대한 지식, 수행, 인식도에 미치는 효과를 파악하고자 시도되었다. 또한 프로그램 적용 후 환자안전 교육에 대한 만족도를 파악한다. 연구의 대상자는 서울 소재 D종합병원 병동에 입원한 환자 30명이다. 2019년 4월 15일부터 4월 30일까지 자료수집 하였으며 자료처리 및 분석방법은 SPSS 23.0을 이용하여 빈도, 백분율, paired t-test 로 분석 하였다. 연구 결과, 모의상황을 활용한 환자안전 교육 프로그램이 입원환자의 환자안전에 대한 지식, 수행, 인식도 향상에 모두가 유의하게 나타났다. 임상환경에서 실제상황과 유사한 모의상황을 경험해봄으로써 입원환자에게 환자 안전에 효과적인 교육 방법임이 확인되었다. 따라서 환자안전 교육 프로그램은 환자의 지식 및 수행, 인식도 향상에 긍정적인 영향을 주며, 임상에서 입원 환자의 환자안전 교육 시 교육 프로그램의 기초로 활용될 수 있을 것이라 기대된다.

As the prevention of patient safety accidents has been strengthened in the accreditation process of medical institutions, patient safety, which is the maintenance of patient safety by managing medical accidents around the patients, is considered a subject that is important as a disease cure. The purpose of this study is to develop a Patient Safety Program using simulated situations for inpatients at a general hospital ward and to understand the effects on knowledge, performance, and perception of patient safety before and after the programs. In addition, the satisfaction of patient safety education is verified after application of the program. The participants were 30 inpatients at a general hospital. Data were collected from April 15 to 30, 2019 and analyzed using IBM SPSS Version 23.0. The results of the preand post-education revealed a statistically significantly improvement of patient safety knowledge, performance, perception and educational satisfaction. The Patient Safety Education Program using simulated situation was an effective educational program for the inpatients to improve patient safety knowledge, performance, perception, and educational satisfaction. Therefore, this program demonstrated a positive effect of patient safety and it is expected that it can be used as the basis of an education program in patient safety education in a clinical setting.

일반인을 대상으로 환자안전 교육을 실시 한 [20], 군 병원 입원환자를 대상으로 교육을 시행 한[14] 연구에서 사용된 환자안전 지식 평가도구를 기반으로 본 연구자가 평가도구를 개발하였으며 10문항 10점 만점으로 구성되었다. 환자안전 지식 문항 개발 시 간호대학 교수 2인과 환자안전에 대한 전문지식을 가지고 있는 전문가 1인, 감염전문가 1인에게 내용타당도를 의뢰한 결과 의미가 명확하지 않은 내용과 중요도가 낮은 문항은 전문가의 조언을 토대로 수정하여 문항을 최종 확정하였다.

대상 데이터

본 연구의 참여자 모집은 병동에 의뢰하여 동의 및 협조를 요청하였고 입원환자들에게 연구의 목적과 방법, 연구 과정과 관련 된 정보 제시하여 연구에 대한 설명을 하고 본인이 자발적으로 참여 의사를 밝힌 환자들로 참여자를 선정하였다. 대상자모집 후 최종 참여자는 총 30명이었다.
본 연구를 위한 교육 요구도 조사는 2019년 02월 07일부터 02월 22일까지 서울 소재 종합병원에 입원하고 있는 환자 20명을 대상으로 설문조사 하였다.

데이터처리

1) 대상자의 일반적 특성과 각 변수의 값은 빈도, 백분율, 평균으로 분석하였다.
2) 교육 전·후에 따른 대상자의 지식, 수행, 인식도는 Paired t-test를 이용하여 검정하였다.
3) 프로그램 중재 후 환자안전 교육에 대한 만족도는 평균 및 표준편차, 빈도분석을 이용하여 분석하였다.

이론/모형

Pyo[20]가 개발한 설문지를 사용 하였으며, 환자안전인식도와 관련된 18개 문항으로 구성되었다. 해당 도구는 의료기관 선택 2문항, 진료과정에서의 환자 참여 6문항, 환자안전사건 예방방법 7문항, 의료분쟁 대응방안 1문항, 환자안전에 대한 전반적 인식 2문항으로 구성되었으며 4점 척도로 4점은 ‘매우 그렇다’, 3점은 ‘그렇다’, 2점은 ‘아니다’, 1점은 ‘전혀아니다’를 의미한다.
본 연구는 환자안전 교육 프로그램 개발을 위하여 교수체제설계의 ADDIE 모형[27]에 근거하여 설계하였다. 프로그램은 분석(analysis), 설계(design), 개발(development), 실행(implementation), 평가(evaluation)의 5단계를 통해 개발한 후 일개 종합병원에 입원하고 있는 환자를 대상으로 적용하여 효과를 측정하기 위한 단일군 전후 실험연구이다.
본 연구에서는 Choi[28]가 개발한 연구도구를 사용하였으며, 8개 부분 37개 문항으로 교수자, 교육내용, 교육 방법, 교육 시설 및 환경, 담당직원의 서비스, 교육훈련기대, 이행성과평가로 구성되었다. 본 연구에서 대상자와 환경을 고려하여 교수자, 서비스, 기대치, 성과를 제외한 강사 1문항, 교육방법 4문항, 교육내용 4문항, 시설 및 환경 1문항으로 교육내용 및 방법을 중심으로 평가하였다.

성능/효과

본 연구 결과 모의상황을 활용한 환자안전 교육 프로그램이 입원환자의 환자안전 지식, 수행, 인식도가 교육전 보다 후에 더 향상되어 효과적인 교육임이 검증되었다. 또한 교육 후 교육 만족도도 높게 나타났다.
임상환경에서 실제상황과 유사한 모의상황을 경험해 봄으로써 입원환자에게 환자 안전에 효과적인 교육 방법임이 확인되었다. 따라서 환자안전 교육 프로그램은 환자의 지식 및 수행, 인식도 향상에 긍정적인 영향을 주며, 임상에서 입원 환자의 환자안전 교육 시 교육 프로그램의 기초로 활용될 수 있을 것이라 기대된다.

후속연구

둘째, 본 연구에서 개발한 병원에 입원한 환자 시나리오 이외에 다양한 모의상황으로 구성 된 시나리오 개발을 제언한다.
첫째, 본 연구에서 개발 된 환자안전 교육 프로그램을 입원 환자가 아닌 다른 대상자에게 적용하여 교육의 효과를 평가하는 연구를 제언한다.
