본 연구의 목적은 요양병원 간호사의 도덕적 민감성, 자기주장성, 윤리적 의사결정 자신감의 정도, 상관관계, 윤리적 의사결정 자신감에 영향을 미치는 요인을 파악하기 위한 서술적 조사연구이다. 연구대상자는 G도에 소재한 6개의 요양병원에 근무하는 간호사 149명으로, 자료수집은 2019년 11월 30일부터 12월 18일까지 시행하였으며 SPSS win 21.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson's correlation 및 Multiple regression으로 분석하였다. 대상자의 도덕적 민감성 정도는 평균 4.94점, 자기주장성 정도는 평균 3.17점, 윤리적 의사결정 자신감 정도는 평균 3.49점이었다. 일반적 특성에 따른 자기주장성 정도는 도덕적 고뇌 경험 유무에서 유의한 차이를 보였고, 윤리적 의사결정 자신감 정도에서는 직위와 윤리적 교육 경험 유무에서 유의한 차이를 보였다. 윤리적 의사결정 자신감은 도덕적 민감성(r=.382, p<.001)과 자기주장성(r=.224, p<.01)과 유의한 정적 상관이 있었다. 요양병원 간호사의 윤리적 의사 결정 자신감에 영향을 주는 변인은 도덕적 민감성과 윤리적 교육 경험 여부, 자기주장성임을 확인하였고, 윤리적 의사결정 자신감에 대한 이들 변인의 설명력은 25.0%였다. 따라서 요양병원 간호사의 윤리적 의사결정 자신감을 향상하기 위해서는 윤리적 교육을 통한 도덕적 민감성과 자기주장성을 함께 향상할 수 있는 프로그램 개발이 필요하다.

This study evaluated the moral sensitivity, self-assertiveness, ethical decision-making confidence and their correlations, and the factors that influence ethical decision-making confidence among nurses working in a long-term care hospital. Overall, 149 nurses working in a long-term care hospital in G Province were evaluated. The date was collected from November 30 2019 to December 18, 2019 and analyzed using SPSS/WIN 21.0 program. The mean of moral sensitivity among nurses was 4.94, the mean of self-assertiveness was 3.17 and the mean of ethical decision-making confidence was 3.49. The self-assertiveness will, according to the general characteristics, was significantly different according to experienced moral distress, and there was a significant difference in the degree of ethical decision-making confidence according to social positions and experience of ethical education. There were positive correlations between the ethical decision-making confidence and moral sensitivity (r=.382, p<.001) and self-assertiveness (r=.224, p<.01). Regression analysis revealed that the factors that significantly influenced ethical decision-making confidence were moral sensitivity, self-assertiveness and ethical education during nursing practice, which explained 25.0% of variance in ethical decision-making confidence. Therefore, to improve ethical decision-making confidence of nurses, programs should be developed that improve moral sensitivity and self-assertiveness by promoting ethical education.

이론/모형

도덕적 민감성은 Lutzen 등[22]의 Moral Sensitivity Questionnaire(MSQ)를 Han 등[23]이 번안하고 수정 보완한 한국판 도구(Korean Moral Sensitivity Questionnaire: K-MSQ)로 측정하였다. 이 도구는 총 27문항으로 구성되었으며 Likert Scale 7점 척도로 5개의 하부영역을 포함한다.
본 연구에서는 Rathus[24]가 개발한 자기주장 도구(Rathus assertiveness schedule)를 Ha 등[25]이 번안하고, Lee & Son[15]의 연구에서 사용한 도구로 측정하였다. 총 30문항으로 구성되었으며 Likert Scale 5점 척도로 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점으로 응답하게 하였으며, 점수가 높을수록 자기주장의 정도가 강한 것을 의미한다.
윤리적 의사결정 자신감 측정도구는 Sulmasy등[26]이 개발한 perceived ethical confidence (self-efficacy) scale (PECS)도구를 Laabs[27]가 수정·보완하고, Kim[10] 이 번역하여 작성한 도구를 사용하였다. 도구는 9개 문항으로 5점 Likert 척도로 ‘매우 그렇다(5점)’에서 ‘전혀 그렇지 않다 (1점)으로, 점수가 높을수록 윤리적 의사결정에 대한 자신감이 높음을 의미한다.

성능/효과

021로 나타나 독립변수들 간 다중공선성의 문제는 없었다. 분석결과 윤리적 의사결정 자신감에 영향을 미치는 변인으로는 윤리적 교육 경험유무(β=-.218, p<.01), 도덕적 민감성(β=.359, p<.001), 자기주장성(β=.170, p<.05)이 영향을 미치는 변인으로 나타났으며, 이 변수들의 총 설명력은 25.0%였다[Table 4].

후속연구

추후 여러 지역의 요양병원 간호사를 대상으로 확대한 연구가 필요하겠다. 또한 요양병원에서의 윤리적인 임상사례를 개발하여 표준화된 대응법 마련을 위한 간호사 윤리지침과 관련된 프로토콜 개발을 제안한다.
본 연구는 요양병원 간호사를 대상으로 윤리적 의사결정 자신감과 자기주장성 간의 상관관계와 영향요인을 파악하기 위해 처음 시도된 연구로 의의가 있으며, 연구의 제한점으로는 G도 지역의 일부 요양병원 간호사를 편의 표집으로 조사한 점으로 연구결과를 일반화하는 데는 주의가 필요하다. 추후 여러 지역의 요양병원 간호사를 대상으로 확대한 연구가 필요하겠다. 또한 요양병원에서의 윤리적인 임상사례를 개발하여 표준화된 대응법 마련을 위한 간호사 윤리지침과 관련된 프로토콜 개발을 제안한다.
