The purpose of this study is conceptual analysis study on the thwarted belongingness, and was based on the conceptual analysis method of Walker & Avant to make clear concept. Thwarted belongingness can be defined by the following attributes: distorted beliefs, insufficient need to belong, a sense of alienation, and insufficient interaction and affection needs. The prerequisites for thwarted belongingness were social isolation, experiences of exclusion or conflict in interpersonal relationships, perfectionist tendencies, and sensitivity to rejection. The consequences occurring as a result of thwarted belongingness were suicidal ideation, decreased happiness and life satisfaction, negative emotions such as anxiety about rejection, depression, shame, loneliness, stress or anger, aggression, hostility and self-criticism, despair, suppression of emotional expression, lethargy, self-exclusion from relationships, and identity confusion. This study is valuable to identify the extent of preventable nursing interventions related to individual mental health and suicidal ideation related to interpersonal relationships, and suggests future tool development and field studies of thwarted belongingness.

본 연구는 Walker와 Avant[11]의 개념분석 방법을 적용하여 좌절된 소속감(Thwarted belongingness) 의속성을 규명하고, 선행요인, 결과요인 및 경험적 준거를 제시하는 개념분석 연구이다. 구체적인 과정은 다음과 같다.
본 연구에서 확인된 좌절된 소속감은 왜곡된 신념, 소속 욕구 불충족, 소외감, 상호교류와 애정욕구의 불 충족으로 확인되었으며 네 가지 속성을 중심으로 논의하고자 한다.

대상 데이터

문헌검색 시기는 2007년부터 2020년 12월까지였으며, 문헌검색을 위해 국내는 한국교육학술정보원 (RISS), 한국학술정보(KISS), 국가전자도서관(NDSL)에서 ‘좌절된 소속감’, ‘대인관계욕구’, ‘자살생각’으로 검색된 학술 및 학위논문을 분석에 사용하였고, 국외는 MEDLINE(PubMed)와 CINAHL, EMBASE을 통해 ‘Thwarted belongingness’, ‘interpersonal theory’, ‘interpersonal needs’, ‘suicidal ideation’으로 검색된 논문을 분석에 사용하였다. 그 결과 809편의 학술논문이 검색되었으며, 중복문헌, 전문이 제공되지 않은 연구, 영어 및 한국어가 아닌 다른 언어로 기술된 연구를 제외하여 280편의 문헌을 대상으로 제목과 초록을 확인하여 본 연구주제에 적합한 54편의 문헌을 분석대상으로 선정하였다. 김슬기씨는 20세 대학생이다. 3명의 친구와 또래집단을 형성하여 학교생활을 해 오던 중 최근 한 친구와 갈등을 겪게 되었다.

이론/모형

좌절된 소속감을 측정하는데 사용되는 도구로서 Van Orden 등[33]이 개발한 대인관계 욕구질문지(Interpersonal Needs Questionaire, INQ)는 ‘좌절된 소속감’과 ‘인식된 짐스러움’을 평가하기 위한 항목으로 구성되어 있으며, 일반적으로 자살생각에 영향을 주는 요인으로 적용되었다. 본 연구의 개념인 좌절된 소속감의 속성에 부분적으로 부합되나 통합적으로 적용되어, 왜곡된 신념, 소속 욕구 불충족, 소외감, 상호교류와 애정욕구의 불 충족으로 분류되지 않는다.

성능/효과

통제력 상실과는 차이가 있었다. 인식된 짐스러움의 속성은 대인관계에서 주로 자신을 향한 주관적 평가의문제이나, 좌절된 소속감은 자신뿐 아니라 타인과의 관계에도 중점을 둔 개념으로 소속감의 개념이 이타적 성향을 포함한 것의 차이로 생각되며, 본 연구를 통해 두 개념은 유사하면서도 구별되는 개념임을 확인할 수 있었다.
좌절된 소속감에 대한 문헌을 고찰한 결과, 좌절된 소속감은 ‘대인관계 상황에서 자신의 왜곡된 신념으로 타인과 연결되고자 하는 욕구가 충족되지 못하여 관계가 단절된 외로운 느낌으로, 타인과의 상호작용의 빈도와 돌봄의 정도가 충족되지 못하는 것이다’로 확인되었다. 구체적인 속성은 다음과 같다.
좌절된 소속감의 선행요인은 사회적 고립, 대인관계에서 배제 혹은 갈등 경험, 개인의 완벽주의적 성향 또는 거부 민감성으로 나타났으며, 이로 인해 발생할 수 있는 결과는 자살생각, 행복 및 삶의 만족 감소, 거부에 대한 불안, 우울, 수치심, 외로움, 스트레스 또는 분노, 공격성, 적대감 같은 부정적 정서, 자기비난, 절망감, 자포자기, 정서표현 억제, 무기력, 관계에서 스스로 배제, 정체성 혼란이다. 우리나라와 같이 타인과의 관계 및 평가를 중요시하는 상호의존적인 문화에서 타인으로부터 애정과 호감을 얻는 것은 중요하며 거부 및 소외의 경험은 매우 두려운 일이 된다[9].

후속연구

좌절된 소속감에 대한 개념은 대인관계욕구 및 자살생각에 있어 중요한 개념이나 통합적 개념으로만 연구되어져, 본 연구를 통해 세부 영역을 확인하여 다른 유사개념과 구별하고 관련 연구의 확장을 도모할 수 있었음에 의의가 있다. 다만, 좌절된 소속감은 청소년기부터 노년기까지 대부분 연령층에 적용될 수 있는 개념이나, 문화권에 따른 해석의 신중함이 필요하며 향후 경험적 준거에 기반하여 대상자별 좌절된 소속감을 측정할 수 있는 도구개발 및 현장연구를 통해본 연구의 속성을 검증해 볼 수 있는 후속 연구를 제언한다.
또한, 이를 통해 대인관계와 관련된 개인의 정신적 건강 및 자살 생각과 관련된 예방 가능한 간호중재의 범위를 파악할 수 있었다는 것에 의의가 있고, 자살예방과 관련된 프로그램 개발시 기초자료로 활용될 수 있기를 기대한다.
자살 고위험군에 대해 미국에서는 사회적지지 부족과 고립감, 관계손실로 수치심 또는 절망을 유발할 수 있는 사건 등 구체적인 전략으로 개인이 고통받을 수 있는 범위의 문제를 다루고 있으나, 우리나라는 자살예방 국가 행동계획에서 고위험군 대상을 언급하고 있을 뿐 자살 고위험군에 대한 국가계획의 미션과 방향이 충분하지 못하였다 [28]. 자살 예방 대책은 자살 고위험군을 조기 발견하고 구체적인 대책마련에 집중할 때 효과적이므로[28], 좌절된 소속감의 개념을 분석하여 속성에 대한 전략을 세우는 것은 향후 자살 고위험군 예방관련 연구에 기반이 될 것이다.
