최근, 비스테로이드성 소염진통제(NSAIDs)의 약물유해반응으로 잘 알려지지 않았던 심혈관계에 대한영향이 밝혀지고 Merck사가 COX-2(cyclo-oxygenase) 선택적 억제제인 로페콕시브(rofecoxib, 상품명은 Vioxx)의 품목허가를 자발적으로 취하하면서 의약품의 안전성에 대한 관심이 다시 증가되고 있다. 뿐만 아니라, 2004년 미국에서 celecoxib와 naproxen 역시 심혈관계 유해반응 발생 위험이 증가될 수 있다는 연구 결과가 보고되면서 NSAIDs에 대한 전반적인 유익성/위험성(risk/benefit)
최근, 비스테로이드성 소염진통제(NSAIDs)의 약물유해반응으로 잘 알려지지 않았던 심혈관계에 대한영향이 밝혀지고 Merck사가 COX-2(cyclo-oxygenase) 선택적 억제제인 로페콕시브(rofecoxib, 상품명은 Vioxx)의 품목허가를 자발적으로 취하하면서 의약품의 안전성에 대한 관심이 다시 증가되고 있다. 뿐만 아니라, 2004년 미국에서 celecoxib와 naproxen 역시 심혈관계 유해반응 발생 위험이 증가될 수 있다는 연구 결과가 보고되면서 NSAIDs에 대한 전반적인 유익성/위험성(risk/benefit) 재평가 필요성이 제기되었다.
따라서 본 연구에서는 NSAIDs의 종합적인 치료적 유익성/위험성 정보를 수집하고, 약물로 인한 위해를 최소화하고 올바른 사용을 유도하기 위하여, NSAIDs의 기본 특성, 작용 기전, 국내외 사용 정황 및 외국의 위해평가 현황 등을 조사하였다. 비스테로이드성 소염진통제(NSAIDs)는, 구조상 코르티코스테로이드 모핵구조를 포함하지 않고 마약성 진통제와 같은 의존성이 없는 약물로, 염증 및 통증 조절용으로 흔히 사용되며 일반적으로 cyclo-oxygenase(COX)를 저해하여 약효를 나타낸다. 국내에는 53개 성분을 함유하는 1,653개 단일제 제품이 허가되어 있으며, 화학 구조 및 COX에 대한 활성 차이에 따라 5개의 약물 계열(acetylsalicylic acid, para-aminophenol 유도체, 비선택적 COX 저해제, COX-2 부분-선택적 저해제, COX-2 선택적 저해제)로 분류하였다. NSAIDs 약물은 주로 114 해열.진통.소염제 효능군에 해당되며 건강 보험심사평가원 자료에 의하면, 2008년도 기준으로 그 사용량이 37억개에 이르는 다빈도 사용 약물군이다.
NSAIDs 경구제 중 다빈도 10순위 성분은, acetaminophen, ibuprofen, aspirin(저용량 아스피린, 분류번호219 해당 제품), talniflumate, loxoprofen, aceclofenac, mefenamic acid, pranoprofen, dexibuprofen, meloxicam 순이었다.
NSAIDs 약물계열별 특성을 조사하기 위하여, 외국과 국내 자발적 부작용 보고 자료 중 NSIADs와 관련된 자료를 추출하여 5가지 약물계열과 NSAIDs에 대하여 잘 알려진 중대한 약물유해반응과의 인과성 및 통계적 유의성을 평가해보았다. WHO 자료에서는, 지금까지 알려진 대로 COX-2 선택적 저해제가 심근경색 및 뇌졸중과 같은 관상동맥 혈전증과 통계적으로 유의한 연관성이 있는 것으로 나타났다. 비선택적 COX 저해제는 소화성 궤양/출혈/천공과 같은 위장관계, 무과립구증과 같은 혈액/림프계, 스티븐스-존슨 증후군/중독성 표피 괴사용해 같은 피부/부속계 유해반응과 통계적으로 유의한 연관성이 있는 것으로 나타났다. 이로부터 자발적 부작용 보고 자료를 활용한 통계분석은, 관심 약물과 특정 유해사례가 signal인지를 조속히 확인할 수 있도록 해주고 약물 계열 또는 약물 계열 내 각 성분별로 특정 유해사례 빈도 변화를 확인할 수 있으므로 시판 후 의약품 안전관리에 있어서 매우 유용한 도구임을 확인할 수 있었다. 국내 부작용 보고 자료의 통계 분석 결과 비선택적 COX 저해제와 스티븐스-존슨 증후군과 통계적 인과성이 있는 것으로 확인되었으나, 이는 자발적 부작용 보고 자료로부터 얻은 실마리 정보로 다른 약물과의 위해성을 직접 비교하는 데에는 한계가 있다. 현재까지 미국 FDA나 유럽 MEA는 수집?평가된 자료에 따라, 치료적 유익성이 위험성을 상회하는 것으로 판단하고 있다. 다만, NSAIDs는 만성관절증에 의한 진통, 소염 효과를 위해 장기적으로 사용될 수 있으며, 일반의약품 해당 제품도 있기 때문에 환자가 부지불식간에 여러 약물을 병용함으로써 일일 최대 권장량을 초과하지 않도록 해야 하며, 의사는 치료적 유익성과 위험성을 충분히 평가하여 신중히 처방할 것을 권고하였다.
또한 외국 의약품 안전관리 규제기관의 NSAIDs 위해평가 결과 및 미국 심장학회나 영국 NICE의 임상가이드라인, 여러 역학조사 결과로부터, 안전한 NSAIDs 사용을 위한 정보를 추출하여 “의·약 전문가용 비스테로이드성 소염진통제의 처방·조제 안내서” 및 “일반 소비자용 비스테로이드성 소염진통제의 사용안내서” 리플렛을 작성하였으며 관련 정보를 국민에게 홍보할 예정이다.

Recently, Merck & Co., Inc. announced a voluntary withdrawal of Vioxx(rofecoxib, a cyclo-oxygenase-2 selective inhibitor) from the U.S. and worldwide market due to safety concerns of an increased risk of cardiovascular events (including heart attack and stroke) in patients on Vioxx. In addition, the National Institutes of Health halted a clinical trial involving NSAIDs in patients at risk of developing Alzheimer's Disease : Preliminary information from the study showed some evidence of increased risk of cardiovascular events, when compared to placebo, to patients taking naproxen. So, re-examination of risk/benefit balance for marketed drugs especially NSAIDs overall was strongly needed.
First, we searched for basic characteristics, mechanism of action, drug utilization data, and risk assessment results of NSIADs. Currently, marketed single NSAIDs(1,653 products) in Korea contains
53 active ingredients. According to pharmacy claimed data in 2008 from Korea Health Insurance
Review Agency(HIRA), frequently used NSIADs were acetaminophen, ibuprofen, aspirin(low dose),
talniflumate, loxoprofen, aceclofenac, mefenamic acid, pranoprofen, dexibuprofen, meloxicam in an
orderly manner. We categorized NSAIDs into 5 classes based on chemical structure and selectivity to
COX(cyclo-oxygenase) as followed: acetylsalicylic acid, para-aminophenol derivatives, non-selective
COX inhibitors, COX-2 preferential inhibitors, COX-2 selective inhibitors.
Secondly, causal relationship analyses between 5 drug classes in NSAIDs and cardiac disorder(myocardial infarction, stroke), nervous system disorders(central nervous system haemorrhages,
cerebrovascular accidents), gastrointestinal disorders(peptic ulcer, ulcer haemorrhage, ulcer perforation,
and ulcer reactivated), blood and lymphatic system disorders(agranulocytosis) and skin & subcutaneous
tissue disorders(Stevens-Johnson syndrome, toxic epidermal necrolysis) using foreign(WHO) and
domestic spontaneous reporting database were performed. Similar to latest results of risk assesments
by US FDA and EMEA, COX-2 selective inhibitors were founded to statistically significant to the
risk of cardiac disorder(myocardial infarction, stroke) and non-selective COX inhibiors were associated
with gastrointestinal disorders(peptic ulcer, ulcer haemorrhage, ulcer perforation, and ulcer reactivated)
and skin & subcutaneous tissue disorders(Stevens-Johnson syndrome, toxic epidermal necrolysis) in
patients prescribed these medicines.
We have concluded that the benefit-risk balance for NSAIDs remains favourable based on currently
available information. But, the overall benefit-risk balance for NSAIDs remains positive when used in
accordance with the product information and taking into account the patient's individual risk
factors(e.g. gastrointestinal, cardiovascular and renal). Consumers are advised that all over-the-counter(OTC) pain medications, including NSAIDs, should be used in strict accordance with the label directions. If use of an OTC NSAID is needed for longer than ten days, a physician should be consulted. Prescribers and patients should continue to use NSAIDs at the lowest effective dose for the shortest possible duration to control symptoms.
Finally considering above-mentioned informations, we made "Guide of NSAIDs for practitioners and
pharmacists" and "Guide of NSIADs for patients" for safe and proper use of NSAIDs.
