본 연구는 노인 고혈압환자의 미충족의료에 영향을 미치는 요인을 조사하는 연구이다. 본 연구의 목적은 65세 이상의 노인 고혈압 환자의 미충족의료에 대한 이유를 파악하고 유형별 특성을 확인해 미충족의료가 발생하지 않도록 의료이용 제도를 개선하고 정책 결정을 하는 데 기여하고자 실시하였다. 국민건강영양조사 제 7기(2016, 2017년) 자료를 이용하여 적절가구 중 계통추출법을 이용하여 23개 표본가구를 선정하였으며 SPSS 18.0 Program을 이용하여 분석하였다. 연구결과 남성보다 여성이, 주관적 건강상태는 나쁠수록, 민간보험에 가입한 사람보다 가입하지 않은 사람, 흡연 경험은 있을 때, 최근 1년간 자살생각이 없었던 사람에 비해 자살생각이 있었던 사람의 미충족의료 경험률이 높게 나타났다 이는 향후 미충족의료가 발생하지 않도록 의료이용 제도를 개선하고 정책 결정을 하는 데 기여할 수 있을 것으로 사료된다.

The purpose of this study is to investigate factors affecting unmet medical care in elderly hypertensive patients. The purpose of this study was to understand the reasons for unmet medical care in elderly hypertensive patients over 65 years of age and to identify the characteristics of each type to improve the medical use system and to contribute to policy making so that unmet medical care does not occur. Using data from the 7th period of the National Health and Nutrition Examination Survey (2016, 2017), 23 sample households were selected using the phylogenetic extraction method among appropriate households and analyzed using the SPSS 18.0 Program. As a result of the study, the unmet medical care of those who had suicidal thoughts compared to those who had not had suicidal thoughts in the past year, when females were higher than males, had worse subjective health status, those who did not subscribe than those who had private insurance, and those who had smoked in the past year. The experience rate was high. This is considered to be able to contribute to improving the medical service system and making policy decisions so that unmet medical care does not occur in the future.

대상 데이터

국민건강영양조사는 전국 규모의 건강 및 영양조사로 조사의 목적은 국민의 건강행태, 건강수준, 식품 및 영양 섭취실태에 대한 통계를 산출해 국민건강증진종합계획의 목표설정 및 평가, 건강증진프로그램 개발 등 보건정책의 기초자료를 활용한 것으로 가구원확인조사, 건강설문조사, 검진 조사, 영양조사를 통해 조사자료를 수집하고 있다. 본 연구는 제 7기(2016, 2017년) 자료를 이용하였으며, 조사구는 192개로 표본조사구 내에서 군대, 양로원, 교도소 등의 시설 및 외국인 가구 등을 제외한 가구 중 계통추출법을 이용해 23개 표본가구를 선정하였다. 표본가구 내에서 적정가구원 요건을 만족하는 만 1세 이상의 모든 가구원을 조사대상자로 선정하였다.

대상자의 일반적인 특성, 사회경제적인 요인, 신체건강 관련요인, 정신건강 관련요인, 질병관련요인, 미충족 의료는 빈도와 백분율을, 각각의 요인들과 혈압관리의 관계를 파악하기 위해 카이제곱 검정(chi-square test)을 시행하였으며, 빈도 백분율, 카이제곱 검정(chi-square test) 분석에서 유의성이 있는 변수들과 미충족 의료에 영향을 미치는 요인을 파악하기 위해 다변량 로지 스틱 회귀분석(multivariate logistic regression analysis)을 시행하였으며, 유의수준은 p <0.05로 하였다.

성능/효과

후속연구

이처럼 기존의 국내 연구는 미충족 의료에 대한 전반적인 인식과 고혈압환자의 혈압조절 관련요인에 대한 분석 이주를 이루고 있어 고혈압 환자의 미충족 의료에 대한 연구는 소수에 그친다. 고혈압과 의료접근성에 취약한 노인을 대상으로 미충족 의료를 조사하는 것이 필요할 것이다.
