This study was conducted to describe the attitude toward pressure ulcer prevention, knowledge and non-compliance risk for pressure ulcer prevention practice and degrees of nursing performance, as well as to examine the relationship among these variables. To accomplish this, we invited 397 nurses from four hospitals to complete surveys of attitude toward pressure ulcer prevention, knowledge and certainty for pressure ulcer prevention practice, and degrees of nursing performance. Non-compliance risk for pressure ulcer prevention practice was derived from the difference between knowledge and certainty for pressure ulcer prevention. The data were collected from March to June 2017 and analyzed by descriptive statistics, independent t-tests, ANOVA, and partial Pearson's correlation coefficient testing. The correct answer rate regarding the knowledge of pressure ulcer prevention practice was 79%, and the degree of pressure ulcer prevention nursing performance was $2.46{\pm}0.31$. The highest item of the non-compliance risk for pressure ulcer prevention practice was incontinence diapers prevent incontinence-associated dermatitis. There were significant negative correlations between non-compliance risk for pressure ulcer prevention practice and knowledge (r=-0.25, p<0.001), and degree of pressure ulcer prevention nursing performance (r=-0.13, p=0.009). Continuous education and furnishing practical guidelines for pressure ulcer prevention should be implemented to improve knowledge and certainty of pressure ulcer prevention practice.

대상 데이터

부산, 경남지역에 소재하는 4개의 2, 3차 의료기관을 편의추출하여 1) 3개월 이상 근무하였고, 2) 본 연구 참여에 동의한 자들을 대상으로 하였다. 근무기간을 3개월 이상으로 설정한 이유는 신규 간호사가 업무파악을 끝낸 후 독립된 간호를 수행할 수 있는 기간을 3개월로 보았기 때문이다.
설문조사는 간호사 10명을 대상으로 우선적으로 내용의 이해와 소요시간 등을 계산하기 위해 예비 조사를 실시한 후 본 조사를 수행하였다. 총 440부를 배부하여 연구참여에 동의하지 않은 38명을 제외한 402부가 회수되었으며(회수율 91.4%) 이 중 응답이 불충분한 자료 5부를 제외한 397부가 분석에 활용되었다.

데이터처리

1) 대상자의 일반적 특성에 따른 욕창예방실무지식과 태도, 욕창예방실무 미준수 위험, 욕창예방간호수행도의 차이는 서술적 통계, independent t-test와 일원배치분산분석 (one-way ANOVA)를 이용하여 분석하였다. 사후검정을 위해서는 Duncan’s test를 실시하였다.
2) 변수들 간의 상관성을 파악하기 위해서는 단변량 분석에서 유의미한 차이를 보였던 일반적인 특성인 성별, 연령, 근무부서, 총 임상경력을 통제한 후 partial Pearson correlation coefficient를 사용하였다.

이론/모형

본 도구는 Kim[23]이 개발한 도구로 근거기반실무에 기초한 욕창발생위험요인 8개 문항과 예방적 간호실무 10개 문항, 2개 영역 18개 문항으로 구성되어있다. 본 연구에서는 하위영역을 나누어 분석하지 않고 하나의 합산 후 평균평점을 활용하여 욕창예방에 대한 실무지식으로 명명하였다.
욕창예방간호수행도를 평가하기 위해서는 Kim[23]의 연구에서 개발된 욕창간호 임상실무 수행도를 측정하는 11개 문항을 적용하였다. 이 도구는 욕창위험사정, 피부관리, 실금/실변관리, 영양과 수분공급, 체위변경, 압력재분산의 여섯가지 영역에서 각 1-2개의 문항에 따른 간호수행이 필요했던 대상자에게 수행한 횟수를 측정하도록 구성되어 있다.
욕창예방에 대한 태도를 측정하기 위해서는 Moore와 Price[21]가 개발한 도구를 Seo [22]가 노인요양시설 간호사를 대상으로 번역, 역 번역한 도구를 사용하였다. 이 도구는 11개 문항으로 구성되었고, 각 문항은 Likert 5점 척도 ‘매우 그렇다’ 1점, ‘그렇다’ 2점, ‘보통이다’ 3점, ‘그렇지 않다’ 4점, ‘전혀 그렇지 않다’ 5점으로 측정하도록 되어 있고 점수가 높을수록 태도가 긍정적인 것을 의미한다.

성능/효과

단변량 분석을 통해 연구대상자들의 일반적 특성에 따라 욕창예방에 대한 태도, 욕창예방실무에 대한 지식, 미준수 위험과 수행도를 살펴본 결과, 근무부서와 총 근무경력에 따라 태도와 욕창예방간호수행도에 차이가 있는 것으로 나타났다. 중환자실이나 재활병동, 내외과계와 같은 부서의 간호사들이 유의하게 욕창에 대한 태도가 긍정적이었고, 이는 관련 연구와 맥을 같이 하며[6] 욕창예방간호 수행도 또한 비교적 높은 것으로 나타났다.

따라서 해석에 있어서 주의를 요하며, 향후 반복적 연구를 통해 재검증 해보아야 할 것으로 제언한다. 둘째, 실제 욕창의 지침을 준수하는지, 얼마나 자주 수행하는지에 대한 조사가 자가보고형식이므로 주관성이 개입할 가능성이 크므로 향후 관찰조사를 통해 타당한 객관성을 확보하려는 노력이 있어야 할 것이다.
첫째, 향후 간호사들의 욕창과 관련된 지식을 측정할 때에는 확신정도를 함께 조사하여 적절한 간호수행이 이루어질 수 있는지에 대해 함께 평가해볼 것을 제언한다. 둘째, 욕창과 관련하여서는 부서마다 지침을 마련한 후 지침수행정도를 조사해볼 필요가 있을 것이다.
