Purpose: To demonstrate the importance of comprehensive skin observation as an effective intervention for pressure injury prevention in elderly long-term care hospital patients. Methods: The survey was conducted with 70 nursing staff members working at two long-term care hospitals with 200 beds or less in D city. Data were collected from October 16 to October 23, 2019 and analyzed using descriptive statistics, the chi-square test, and the independent t-test with the SPSS 25.0 program. Results: Nursing care knowledge for pressure injury was similar between the two groups. Conversely, nursing care performance for pressure injury degree of the nursing staff in the hospital using skin observation records performed better than those who did not (t=6.11, p<.001). Furthermore, comprehensive skin assessments in long-term care hospitals using skin observation records showed a lower incidence rate of pressure injury than that showed using general skin assessments (t=-5.28, p=.006). Conclusions: Comprehensive skin assessment is important for pressure injury prevention in elderly long-term care hospital patients. To implement this effectively, it is necessary to devise institutional regulations, guidelines, and systematic education programs.

욕창간호 수행도는 2013년 병원간호사회에서 개발하고 2018 년 개정한 욕창간호 실무지침[11]에서 요양병원에 해당되는 내용으로 연구자가 수정하여 구성한 23문항의 욕창간호 수행도 측정도구를 사용하였다. 도구는 욕창발생위험의 사정, 피부와 욕창 사정, 피부간호, 실금과 실변 관리, 영양관리, 자세변경, 압 력 재분산(지지면 활용), 감염예방 등으로 문항을 구성하였다.
욕창간호 지식은 2013년 병원간호사회에서 개발하고 2018년 개정한 욕창간호 실무지침[11]에서 요양병원에 해당되는 내용으로 연구자가 수정하여 구성한 20문항의 욕창간호 지식 측정 도구를 사용하였다. 도구는 욕창발생위험의 사정, 피부와 욕창 사정, 피부간호, 영양관리, 자세변경, 압력 재분산(지지면 활용), 상처세척, 드레싱, 감염예방 등으로 문항을 구성하였다.

대상 데이터

연구대상자는 D시에 위치하고 있는 200 병상 이하인 요양병원 두 기관에 근무하는 만 19세 이상의 간호 인력 중 연구 참여에 자발적으로 동의한 자로 선정하였고, 입사 3개월 미만인 자는 수습기간이기 때문에 연구대상에서 제외하였다 . 간호 인력이란 의료기관별 환자 인당 간호사 수를 의미하며, 요양병원, 의원의 경우 간호사 정원 중 일부를 간호조무사로 충당할 수 있도록 허용하고 있다 [18].
효과크기는 간호사를 대상으로 한 욕창 교육프로그램의 효과를 연구한 선행연구를 근거로 하였다 [20]. 74명에게 수집한 자료 중 모든 문항에 답하지 않은 미완성 자료를 제외 하여 두 기관 각각 35명, 총 70명의 자료를 최종 분석에 사용하였다.

데이터처리

수집된 자료는 SPSS/WIN 25.0 프로그램을 이용하여 전산통계 처리하였고, 연구대상자의 일반적 특성, 욕창간호 지식, 수행도, 욕창발생률은 기술통계분석을 통하여 빈도, 백분율, 평균 및 표준편차를 구하였다. 대상 집단 간의 동질성 검정 및 피부 관찰기록지 사용여부에 따른 욕창간호 지식, 수행도, 욕창발생률 의 차이는 chi-square test, independent t-test 분석을 실시하였다.
0 프로그램을 이용하여 전산통계 처리하였고, 연구대상자의 일반적 특성, 욕창간호 지식, 수행도, 욕창발생률은 기술통계분석을 통하여 빈도, 백분율, 평균 및 표준편차를 구하였다. 대상 집단 간의 동질성 검정 및 피부 관찰기록지 사용여부에 따른 욕창간호 지식, 수행도, 욕창발생률 의 차이는 chi-square test, independent t-test 분석을 실시하였다.

성능/효과

본 연구는 피부관찰기록지를 이용하고 있는 요양병원과 이용하지 않는 요양병원 간호 인력의 욕창간호에 관한 지식과 수행도 및 욕창발생률을 비교해봄으로써 추후 요양병원에 입원하는 노인대상자의 효율적인 욕창예방중재의 일환으로 포괄적 피부사정의 중요성에 대한 근거를 마련하기 위해 실시하였다. 연구 결과 두 집단 간 욕창간호에 관한 지식 정도는 차이가 없었지만 피부관찰기록지를 이용하는 병원의 간호 인력이 이용하지 않는 병원의 간호 인력에 비해 욕창예방 수행도가 더 높았고, 피부관찰기록지를 이용하는 요양병원의 욕창발생률은 더 낮은 것으로 나타났다. 이는 간호 인력에게 피부관찰기록지를 제공 하여 포괄적 피부사정을 실시하도록 하는 것이 간호 인력의 개인적 특성에 따라 일반적으로 시행하는 피부사정에 비해 욕창 예방에 있어 효과적이라고 할 수 있다.

후속연구

본 연구 결과를 바탕으로 다음의 추후 연구를 제언한다. 첫째, 요양병원 실정에 맞는 보다 전문적이고 근거 있는 욕창 지식 및 수행도를 측정할 수 있는 도구의 개발이 필요하다. 둘째, 대상자를 확대하여 피부관찰기록지를 이용한 포괄적 피부사정이 욕창발생률 감소에 효과가 있는지 반복 연구가 필요하다.

	대형병원보다 요양병원에서 욕창이 많이 발생하는 이유는?	대형병원에 비해 간호사의 배치수준이 열악한 요양병원에서는 간호사들의 직접간호 시간보다 보조인력 위주의 환자 돌봄이 많고[14], 간호사가 피부문제나 욕창을 먼저 발견하기 보다는 필요에 따라 행해지는 체위변경, 기저귀 교체, 또는 목욕하는 과정에서 간호조무사나 간병사에 의해 피부문제가 발견되기도 한다. 요양병원에 입원한 노인대상자의 욕창예방간호를 간호사가 모두 직접 수행하는 것은 무리가 있고, 대다수의 요양병원에서는 간호 인력에 포함되어 있는 간호조무사에게 간호행위의 상당부분을 위임하고 있는 실정으로[15,16] 노인대상자를 위한 욕창예방활동이 효율적으로 제공되기는 어려운 실정이다.
